El absceso renal es una entidad poco frecuente en pediatría. Objetivo: reportar presentación clínica, diagnóstico y terapia en una cohorte de 20 niños con absceso renal. Pacientes y Método: estudio retrospectivo de casos de absceso renal durante un periodo de 10 años en el Hospital Roberto del Río. Se analizaron las características clínicas, de laboratorio, imagenológicas y el tratamiento realizado. Resultados: 20 casos, 65% mujeres, mediana de edad de 3,6 años. La presentación clínica más frecuente fue fiebre, vómitos y síntomas urinarios bajos. El 80% presentó elevación de parámetros inflamatorios y 88% urocultivo positivo, siendo el microorganismo más frecuente Escherichia coli (77,8%). El diagnóstico se realizó mayoritariamente con ecografía renal. El 95% requirió solo tratamiento antibiótico. Se pesquisó reflujo vesicoureteral en 28% de los pacientes sin control de esfínter. En aquellos pacientes con control de esfínter el diagnóstico de disfunción vésico-intestinal estuvo presente en 90% de los casos. El 44% de los pacientes a quienes se les realizó cintigrama renal DMSA tardío presentaron cicatriz renal. Conclusiones: El absceso renal en esta serie se presentó principalmente con fiebre persistente pese al tratamiento, requiriendo terapia antibiótica prolongada, y rara vez siendo necesario el drenaje quirúrgico. Sugerimos realizar estudio destinados a detectar factores modificables, como reflujo vésico-ureteral en pacientes sin control de esfínter, disfunción vésicointestinal en pacientes con continencia, y búsqueda activa de secuelas parenquimatosas renales.
