The pooled estimate of total mortality from rAAA is very high, although it has declined over the years. Most patients die outside hospital, and there is no surgical intervention in a considerable number of those who survive to reach hospital.
In vascular surgery patients, preoperative cognitive impairment and open aortic or amputation surgery were highly significant risk factors for the occurrence of POD. In addition, POD was significantly associated with a higher mortality and more institutionalization. Patients with these risk factors should be considered for high-standard delirium care to improve these outcomes.
This meta-analysis suggests the diagnostic performance of WBC scintigraphy combined with SPECT/CT is the greatest in diagnosing VGI. However, it is a time consuming technique and not always available. Therefore FDG-PET/CT may be favourable as the initial imaging technique. The use of solitary CTA in diagnosing VGI seems to be obsolete.
ObjectiveAlthough the guiding principle of clinical governance states that healthcare professionals are the leading contributors to quality and safety in healthcare, little is known about what healthcare professionals perceive as important for clinical governance. The aim of this study is to clarify this by exploring healthcare professionals' views on clinical governance.DesignBased on a literature search, a list of 99 elements related to clinical governance was constructed. This list was refined, extended and restricted during a three-round Delphi study.Setting and participantsThe panel of experts was formed of 24 healthcare professionals from an academic hospital that is seen as a leader in terms of its clinical governance expertise in the Netherlands.Main outcome measuresRated importance of each element on a four-point scale.ResultsThe 50 elements that the panel perceived as most important related to adopting a bottom-up approach to clinical governance, ownership, teamwork, learning from mistakes and feedback. The panel did not reach a consensus concerning elements that referred to patient involvement. Elements that referred to a managerial approach to clinical governance and standardisation of work were rejected by the panel.ConclusionsIn the views of the panel of experts, clinical governance is a practice-based, value-driven approach that has the goal of delivering the highest possible quality care and ensuring the safety of patients. Bottom-up approaches and effective teamwork are seen as crucial for high quality and safe healthcare. Striving for high quality and safe healthcare is underpinned by continuous learning, shared responsibility and good relationships and collaboration between healthcare professionals, managers and patients.
To our knowledge, these are the first type III endoleaks reported in association with Ancure endografts. Placement of Wallstents inside endografts is of concern, and another indication for close monitoring during follow-up.
ࡗ ࡗPurpose: To evaluate the magnetic resonance imaging (MRI) characteristics of commercially available stent-grafts used for abdominal aortic aneurysm repair. Methods: Seven endovascular grafts (AneuRx, Lifepath, Talent, Excluder, Zenith, Quantum LP, and Ancure) were suspended in a water bath containing gadolinium and scanned using a 1.5-T clinical MRI scanner. Two different scan techniques (T 1 -weighted spoiled gradient echo and spin echo) based upon a clinical MRI endograft surveillance protocol were used for each stent-graft. The scans were evaluated for susceptibility artifacts and radiofrequency (RF) shielding and caging artifacts. Results: For most endografts, the lumen and structures surrounding the endograft were well visualized. However, the ferromagnetic properties of the Zenith and Lifepath devices resulted in large susceptibly artifacts that obliterated the endograft lumen as well as adjacent structures. All fully supported grafts showed some amount of signal loss from the graft lumen caused by RF caging. For the Ancure graft, evaluation around the attachment sites might be problematic. Conclusions: An MRI-based surveillance protocol appears to be a viable option for the AneuRx, Talent, Excluder, and Quantum LP stent-grafts.
Low overall transient and permanent SCI rates are achieved during endovascular thoracic and thoraco-abdominal aortic repair. Based on the presented data, the use of selective spinal fluid drainage in high risk patients seems justified. Peri-operative hypotension should be avoided and treated where possible. The use of mild hypothermia is promising in small cohorts, but requires further evaluation. Further high quality data are essential to establish a definitive preventive strategy.
ObjectiveBiomechanics for rupture risk prediction in abdominal aortic aneurysms (AAA) are gaining popularity. However, their clinical applicability is still doubtful as there is lack of standardization. This study evaluates the added value of biomechanical indices in rupture risk assessment.MethodsThis study included 175 asymptomatic, 11 sAAA and 45 ruptured aneurysms. 3D-geometries were reconstructed using computer tomography angiographies. Subsequently, finite element models were made to calculate peak wall stress (PWS), peak wall rupture index (PWRI) and the rupture risk equivalent diameter (RRED). The indices were determined with a dedicated software to facilitate standardization.ResultsSAAAs showed a trend towards higher PWS, PWRI and RRED compared to asymptomatic AAAs, but PWS (22.0±5.8 vs. 33.4±15.8 N/cm2), PWRI (0.52±0.2 vs. 1.01±0.64), and RRED (65±60 vs. 98±51 mm) were significantly (p = 0.001) higher in ruptured. However, after diameter-matching no significant differences were seen. The ROC-curves for the maximum diameter and all biomechanical indices were similar but it slightly increased when diameter and biomechanical indices were combined.ConclusionsThis study showed no added value for biomechanical indices in AAA rupture risk assessment. Additionally, the difficulty of such an assessment increases. However, as symptomatic aneurysms show a trend towards higher biomechanical indices with similar diameters the indices may provide information about aneurysm growth and development.
