A prospective randomized clinical trial was conducted on 162 patients of osteoarthritis of knee were included in the study. The patients were divided into two groups-Group A and Group B. The Group A was treated with shortwave diathermy, exercise, naproxen and activity modification and the Group B was treated with shortwave diathermy, exercise and naproxen. Improvement was found more in Group A than Group B after 4 th week (95 % CI was -2.59 to 6.56). Then it was found that the improvement was gradually increased in Group A than Group B and finally, it was found that there was highly significant improvement in Group A than Group B after 6 th week (95 % CI was -3.45 to -0.70). This study suggests that activity modification play an important role for the treatment of the patients with osteoarthritis of knee.
Background: Cervical spondylosis is the results of disc degeneration with associated osteophytosis. Cervical traction is widely used to treat neck disorders. Rehabilitation treatment like cervical traction along with other measures at home may play an important role to reduce symptoms of the patients with cervical spondylosis.
Objective: A randomised clinical trial was conducted among 125 patients having cervical spondylosis to find out the effects of manual continuous home cervical traction.
Methods: The patients were selected on the basis specific selection criteria. They were subdivided into two groups. In Group-A (manual continuous home cervical traction group), 61 patients were treated with exercise, cervical collar, neck support, manual continuous home cervical traction, NSAID, warm moist compression and instruction in posture. In Group-B (conventional treatment receiving group), 64 patients were treated with exercise, cervical collar, neck support, warm moist compression, NSAID and instruction in posture.
Results: Among the subjects there were 68 (54.4 %) male and 57 (45.6 %) female. The mean age of the subjects was 45.94 ± 11.65 years. There was marked improvement of symptoms of the patients of Group-A in response to treatment for 6 weeks (p < 0.007). It indicates that manual home cervical traction was found effective to reduce the sign and symptoms of cervical spondylosis. There was improvement after treatment in Group-B also (p < 0.01). It indicates that conventional treatment was also found effective. In comparison, there was no significant difference between two groups (p < 0.36, 95% CI= ‐0.56 to 1.51) found after first week. But there was significant improvement in Group-A than Group-B after six weeks of treatment (p < 0.003, 95% CI= -2.40 to -0.51).
Conclusion: It may be concluded that manual continuous home cervical traction is beneficial for the patients with cervical spondylosis.

Disorders of musculo-skeletal (MSK) system are common conditions seen in diabetic patients. The present study investigated the pattern of MSK disorders among the Bangladeshi diabetic patients attending the Department of Physical Medicine and Rehabilitation of a tertiary care hospital in Dhaka over a period of one year. A total of 2062 diabetic patients with MSK disorders were included in the study. Degenerative joint diseases were more common (53.2%) than inflammatory arthropathies (20.1%).Ibrahim Med. Coll. This prospective experimental study was done in the Department of Physical Medicine and Rehabilitation, Bangabandhu Sheikh Mujib Medical Universiry, Dhaka from 1st January 2008 to 31st July 2008 to observe the effects of ADL instructions on patients with osteoarthritis of the knee. A total of 60 patients with OA – knee were selected. Out of 60 patients 28(46.7%) were female and 32(53.3%) were male. The female to male ratio1.14:1. Age distribution varied from >40 years to <70 years with a mean age was 53.58 ± 6.86 years. Maximum patients 17(28.3%) belonged to age group of 56-60 years in both sexes and house wives were maximum in number 26 (43.3%).They were divided into two groups, 30 patients treated with therapeutic exercise plus NSAID and another 30 patients were with therapeutic exercise, NSAID and ADL. In both groups patients were treated for six weeks. In the present study, the group of patients who received NSAID, Exercise and ADL instructions improved more significantly than those who were not advised ADL instructions but received NSAID and exercise only (95% CI: 4.30 to 9.36, P = 0.001) .So, from the present study it may be concluded that activities of daily living instructions play a role to improve the symptoms of OA knee.
