Zusammenfassung Einleitung: Das Compliance Inventar (CI−6) bietet eine für klini− sche und Forschungszwecke standardisierte Möglichkeit, die Be− handlungscompliance stationärer Patienten mit schizophrenen und schizoaffektiven Störungen retrospektiv zu erfassen. Die umfassende Behandlungscompliance wurde definiert als freiwil− lige und einsichtige Teilnahme an den medizinischen und psy− chosozialen Behandlungsempfehlungen. Methode: Das CI−6 be− steht aus insgesamt 6 Items zur Einsicht, Freiwilligkeit und Teil− nahme an den medizinischen und psychosozialen Behandlungs− empfehlungen. Für eine Stichprobe von n = 52 Patienten mit schizophrenen und schizoaffektiven Störungen wurden die Inter− rater−Reliabilität bestimmt, erste Validitätsberechnungen vorge− nommen und die empirische Überlappung von Einsicht, Freiwil− ligkeit und Behandlungsteilnahme untersucht. Ergebnisse: Die Interrater−Reliabilität ergab eine gute Übereinstimmung bei al− len 6 Items (kappa jeweils > 0,75). Bessere Behandlungscompli− ance korrelierte signifikant mit höherem Alter, weiblichem Ge− schlecht, schizoaffektiver Störung, freiwilliger Krankenhausauf− nahme sowie dem klinischen Gesamteindruck bei Entlassung. Zwischen Einsicht, Freiwilligkeit und Behandlungsteilnahme be− standen substanzielle aber keinesfalls vollständige Überlappun− gen. Insbesondere freiwillige Behandlungsteilnahme trotz feh− lender Einsicht war relativ häufig. Schlussfolgerung: Nach der Interrater−Übereinstimmung ist das CI−6 ein reliables Instrument zur retrospektiven Erfassung der Behandlungscompliance von stationär aufgenommenen Patienten mit schizophrenen Störun− gen. Für viele klinische und wissenschaftliche Fragestellungen erscheint uns diese quantitative Erfassung der Behandlungscom− pliance besser geeignet zu sein als die übliche dichotome Unter− scheidung zwischen Patienten mit complianter und nicht−com− plianter Medikamenteneinnahme.
