Hintergrund und Ziel: Das Descending-Perineum-Syndrom (DPS) wurde 1966 als Synonym für eine komplexe Beckenbodenfunktionsstörung beschrieben. In der Diagnostik von Beckenbodenfunktionsstörungen steht heute eine Reihe von Untersuchungsmethoden zur Verfügung. Bei der Vielzahl diagnostischer Verfahren ist eine gut geplante Stufendiagnostik wichtig, um bei geringem Ressourcenverbrauch alle notwendigen Informationen zu erhalten. Das Ziel dieser Studie war, den Stellenwert der Manometrie in der Diagnostik des DPS zu untersuchen.Patienten und Methodik: Für die Untersuchung wurden Patientinnen mit klinischem und radiologischem Nachweis eines DPS mit einer gesunden Kontrollgruppe verglichen. Wäh-rend des standardisierten Untersuchungsablaufs wurden nach ausführlicher Anamnese mit Erhebung eines Inkontinenzscores eine digitale Untersuchung, eine endoanale Sonographie und eine neurophysiologische Untersuchung mit Bestimmung der motorischen Latenzzeit des Nervus pudendus (PNTML) sowie Prüfung des anorektalen Reflexes durchgeführt. Im gleichen Untersuchungsgang erfolgte dann die Vektorvolumenmanometrie als Perfusionsdurchzugsmanometrie.Ergebnisse: Insgesamt wurden 72 Patientinnen untersucht, 47 Frauen in der DPS-Gruppe und 25 in der Kontrollgruppe. Der Vergleich der Ruhedrücke ergab keinen signifikanten Unterschied (p = 0,053), wobei aber der Mittelwert des Ruhedrucks bei den DPS-Patientinnen niedriger lag. Auch die Kneifdruckwerte waren zwischen den Gruppen nicht signifikant unterschiedlich. Der Vergleich der Vektorvolumenmanometrie zeigte beim Ruheund beim Kneifvektorvolumen ebenfalls keine signifikanten Differenzen. Die funktionelle Sphinkterlänge beim Kneifen war in der DPS-Gruppe signifikant geringer. Auch die Sphinkterlängendifferenz war zwischen den Gruppen hochsignifikant unterschiedlich (p < 0,001); beim Kneifen kam es in der DPS-Gruppe zu einer signifikanten Verkürzung der Hochdruckzone (p < 0,01).Schlussfolgerung: Die Konstellation mit Abnahme der funktionellen Länge des Sphinkters und der Hochdruckzone beim Kneifen und erniedrigter Sphinkterlängendifferenz ist ein deutlicher Hinweis auf ein DPS. Die Vektorvolumenmanometrie kann ein wichtiger diagnostischer Schritt vor der weitergehenden apparativen Diagnostik sein.
The treatment of acute life threatening conditions normally requires intravenous access for further therapeutic measures. In the current guidelines of the European Resuscitation Council (ERC), published in November 2005, the significance of intraosseous access had been newly regulated. Nevertheless two years after publication of these guidelines, intraosseous acccess in adults is still only implemented sporadically. The translation of scientific knowledge into clinical practise takes several years as a general rule. The following article illustrates the basic literature and two case reports, and is designed to inspire trust in a method, which can provide a useful alternative to peripheral venous access for the emergency physician.
