Endokrinologische Notfälle machen nur einen kleinen Teil der gesamten Notfallmedizin aus, sind aber insbesondere dann lebensbedrohlich, wenn die Diagnose verzögert gestellt wird. Daher ist es wichtig, die Diagnose frühzeitig anhand des klinischen Bildes zu stellen. Die Therapie sollte dann sofort beginnen, ohne dass Laborergebnisse abgewartet oder apparative Untersuchungen durchgeführt werden. Im Folgenden soll auf Pathogenese, Diagnostik und Therapie von diabetischem Koma, akuter Nebenniereninsuffizienz und thyreotoxischer Krise eingegangen werden. Diabetisches Koma (Ketoazidose und hyperosmolares Coma diabeticum) ! Die diabetische Ketoazidose und das hyperosmolare Coma diabeticum treten meist bei Patienten mit bekanntem Diabetes mellitus auf, können aber auch die Erstmanifestation eines Diabetes darstellen. Vor der Entdeckung des Insulins betrug die Mortalität der diabetischen Ketoazidose 90 %. Durch den Einsatz von Insulin, Antibiotika und forcierter Rehydratation konnte sie auf weniger als 20 % gesenkt werden. Mit einer leitliniengerechten Therapie liegt die Mortalität heute unter 2 % [1, 2], wobei sie jedoch bei älteren Patienten und Patienten mit schweren Begleiterkrankungen höher ist. Definition Die diabetische Ketoazidose ist gekennzeichnet durch die Trias Hyperglykämie, Azidose und Ketonämie. Der Übergang von einer schweren Ketoazidose in ein Coma diabeticum ist graduell und hängt davon ab, wie stark die Serumosmolalität steigt. Normoglykämische diabetische Ketoazidosen wurden mit einer Häufigkeit von bis zu 18 % beschrieben [3]. Entsprechend den Leitlinien der Deutschen Diabetesgesellschaft (DDG) [4] ist eine diabetische Ketoazidose definiert durch:" Blutglukose ≥ 300 mg/dl (16,7 mmol/l) " BE -20 mmol/l " Standardbikarbonat < 15 mmol/l " Betahydroxybutyrat > 3,0 mmol/l " pH < 7,3 " Ketonurie, Erbrechen und eingeschränktes Bewusstsein In der EURODIAB-Studie wurde die Inzidenz der diabetischen Ketoazidose bei Typ-1-Diabetikern mit 8,6 % angegeben [5]. Lange Zeit nahm man an, dass das ketoazidotische Koma pathognomonisch für den Diabetes mellitus Typ 1 ist. In den letzten Jahren wurden aber mehrere Fälle von diabetischer Ketoazidose bei Patienten mit Typ-2-Diabetes beschrieben, vor allem bei Afroamerikanern und ethnischen Minderheiten [6,7]. Glukose kann nur in ausreichendem Maß in die Zellen aufgenommen und zur Energiegewinnung genutzt werden, wenn Insulin vorhanden ist. ZusammenfassungUnter dem Begriff Lipodystrophie wird eine Gruppe angeborener und erworbener Erkrankungen subsummiert, denen gemeinsam der Verlust an subkutanem Fettgewebe ist. Dieser Fettgewebsverlust kann den gesamten Körper (generalisierte Lipodystrophie) oder nur Teile davon (partielle Lipodystrophie) betreffen. Durch das fehlende Fettgewebe kommt es zu einer Dysbalance adipozytensezernierter Faktoren, wobei insbesondere der Minderproduktion von Adiponektin und Leptin eine wesentliche pathophysiologische Rolle zukommt. Es resultieren schwere Fett- und Glukosestoffwechselstörungen mit entsprechenden Folgeerkrankungen, welche die Patienten vital gefährden können. Oftmals ist unter konventioneller Therapie nur eine unzureichende Stoffwechselkontrolle möglich. Durch die rekombinante Herstellung von Leptin ist es gelungen, betroffene Patienten einer kausalen Therapie zuzuführen.
