In dieser Studie wurde der Haltetremor der Hände bei 20 Patienten mit idiopathischer Parkinson-Erkrankung untersucht. In 16 Fallen lag em bilateraler Haltetremor vor. Klinisches Rating und Spektralanalysen von EMG und Akzellerometrie wurden mit und ohne Gewichtsbelastung der tremolosen Extremität durchgefUhrt. Alle Messungen wurden jeweils vor und nach Gabe von Levodopa vorgenommen. Durch die akzellerometrischen Untersuchungen Iie&en sich verschiedene Formen von Haltetremor unterscheiden. An 27 Händen fand sich em mit der Frequenz des jeweiligen Ruhetremors oszillierender Haltetremor (lyp I), an 9 Händen war em nicht mit Ruhetremor assoziierter bzw. mit anderer Frequenz schlagender Haltetremor (Typ II) feststellbar. Der Tremor lyp II lieg sich weiter in Tremores ohne (5 Hände) und mit (4 Hände) peripherer Komponente differenzieren. Tremores mit peripherer Komponente zeigten em Absinken der Peak-Frequenz unter Gewichtsbelastung und zum Teil fehlende EMG-Synchronisation. Alle Formen von Haltetremor zeigten em gutes Ansprechen auf die Gabe von Levodopa.In the present study postural tremor of the hands was assessed in 20 patients with idiopathic Parkinson's disease. In 16 cases postural tremor was bilateral. Clinical rating and spectral analysis of accelerometry and electromyography were performed with and without loading of the limb. All assessments were done before and after administration of levodopa. Accelerometry disclosed different types of postural tremor. In 27 hands a postural tremor oscillating in the frequency-band of rest-tremor was found (Type I). In 9 hands postural tremor occurred without corresponding rest tremor or with a different peak frequency (Type II). Type II tremor was further differentiated according to the presence (4 hands) or absence (5 hands) of peripheral components. Tremors with peripheral components showed slowing of peak frequency with weight load and partly lack of EMG synchronisation. All types of postural tremor responded well to levodopa challenge.
