ZusammenfassungAngeborene Stoffwechselerkrankungen sind insgesamt selten, die kumulative Inzidenz beträgt jedoch ca. 1 : 1600–2000 Lebendgeborene. Sie können sich in jedem Lebensalter manifestieren. Die Symptomatik ist häufig unspezifisch, der Zustand der Patienten in der akuten Stoffwechselentgleisung nicht selten kritisch. Die Langzeitprognose hängt in entscheidendem Maße von der raschen Diagnosestellung und Einleitung der spezifischen Therapie ab.Bei einem kritisch kranken Kind mit unklarer Ursache muss differenzialdiagnostisch immer ein Stoffwechseldefekt erwogen werden. Hier sollte zunächst eine Stoffwechsel-Basisdiagnostik durchgeführt werden. Diese umfasst eine Blutgas-Analyse, die Bestimmung von Blutzucker, Laktat und Ammoniak sowie einen Urinstatus inklusive Ketonkörper. Verstärken sich hierbei Hinweise auf das Vorliegen einer Stoffwechselerkrankung, erfolgt im zweiten Schritt die erweiterte Stoffwechseldiagnostik, bestehend aus Aminosäurenanalyse in Trockenblut und Plasma, Analyse von Acylcarnitinen und freiem Carnitin im Trockenblut, Bestimmung von freien Fettsäuren und Ketonkörpern im Serum sowie Analyse der organischen Säuren im Urin.Anhand der metabolischen Leitsymptome Hyperammonämie, Laktatazidose und Hypoglykämie orientiert sich das weitere diagnostische und therapeutische Vorgehen.Wichtigster therapeutischer Schritt bei Verdacht auf Bestehen einer Stoffwechselerkrankung ist die Sicherstellung einer adäquaten Energiezufuhr. Alle weiteren diagnostischen und therapeutischen Schritte sollten mit dem nächstgelegenen Stoffwechselzentrum abgesprochen werden. Eine frühzeitige Verlegung des Patienten in ein Stoffwechselzentrum ist unbedingt anzustreben.Wenn ein Kind aus ungeklärter Ursache plötzlich verstirbt, sollte immer eine angeborene Stoffwechselstörung in Betracht gezogen werden. Eine sorgfältige postmortale Diagnostik erlaubt eine genetische Beratung der Familie und eine Risikoabschätzung für weitere Schwangerschaften. Hierzu ist es notwendig, die entsprechenden Proben zu asservieren. Dazu gehören Serum und Plasma, eine Trockenblutkarte, Urin, Liquor und Gewebeproben (Haut zur Anzucht von Fibroblasten, Leber, Muskel, ggf. Herzmuskel).
