Die intraoperative Erfassung der Kontraktilität des Herzens ist mit den bisher zur Verfügung stehenden Diagnostika nicht klinisch durchführbar. Zur Beurteilung des chirurgischen Traumas am Myokard selbst bleibt die Beobachtung der postoperativen hämodynamisehen Pumpleistung des Herzens. Die wachsende Bedeutung der Koronarchirurgie macht die Entwicklung einer schon intra· operativen Funktionsanalyse des Myokards besonders dringend, da so die Wirksamkeit eines koronaren Bypasses kontrolliert werden könnte. Die Messung der Lateralkompressionskraft in einem eng begrenzten Areal geringgradig gespreizter Myokardfasern hat sich zunächst im Tierversuch an 83 Hunden als Verfahren zur kontinuierlichen Kontrolle der lokalen Myokardkraft bewährt. Es eignet sich zu Langzeitmessungen über mehrere Wochen, es ist mit einem geringen Trauma am Myokard selbst verbunden, und es ist intraoperativ leicht anwendbar ohne den eigentlichen chirurgischen Eingriff zu behindern. Der von uns entwickelte Meßkopf besteht aus einem intramyokardialen Doppelstift, der die Lateralkompressionskraft eines aufgespreizten Muskelbündels mißt. Ein 3 mm dikdes und 8 mm langes Rohr, das sich in 4 bzw. 7 mm Abstand von der Basis zu einem querovalen Fenster öffnet, enthält einen inneren Stift, der senkrecht auf einer Membran steht. Durch das seitliche Fenster legen sich dem inneren Stift Myokardfasern an und bewirken nach Maß ihrer Längsspan-nung eine Deflektion des inneren Stiftes. Dadurch wird die basale Membran S-förmig Reformiert. Dehnmeßstreifen (Kulite Sensors Ltd, Type WEP-: 350-060), die auf die Membran aufgeklebt sind (m-bond 600 adhesive, Micro Measurements Vishag Intertechnology Inc.), nehmen diese Verformung auf. Sie sind als Halbbrücke geschaltet. Ihre Anordnung auf der Membran läßt nur bei einer S-förmigen Auslenkung der Membran ein Brückensignal entstehen.
