The healthy action of probiotics is not only due to their nutritional properties and their influence on the gastrointestinal environment, but also to their action on the immune system. The aim of the present study was to determine if 6 weeks of probiotic intake would be able to modulate the immune system in women who had recently delivered and were breast-feeding. The design consisted of a randomised, controlled and double-blind nutritional intervention study with parallel groups with a sample size of 104 women. The main variable is the T helper type 1/T helper type 2 (Th1/Th2) profile determined by measuring interferon-g (Th1) and IL-4 (Th2) values in peripheral blood by flow cytometry. The modifications of cytokines were evaluated in maternal milk by cytometric bead array in a flow cytometer and ELISA at three stages of breast-feeding: colostrum, early milk (10 d) and mature milk (45 d). Additionally, the anthropometry and infectious and allergic episodes in the newborn were followed up throughout the first 6 months of life. After the consumption of milk fermented with Lactobacillus casei during the puerperium, we observed a nonsignificant increase in T and B lymphocytes and a significant increase in natural killer cells. A decrease in the pro-inflammatory cytokine TNF-a in maternal milk and fewer gastrointestinal disturbances were also observed in the breast-fed child of the mothers who consumed L. casei. The intake of milk fermented with L. casei during the lactation period modestly contributes to the modulation of the mother's immunological response after delivery and decreases the incidence of gastrointestinal episodes in the breast-fed child.

Hantavirus pulmonary syndrome (HPS) is the most frequently reported fatal rodent-borne disease in Brazil, with the majority of cases occurring in Santa Catarina. We analysed the clinical, laboratory and epidemiological data of the 251 confirmed cases of HPS in Santa Catarina in 1999-2011. The number of cases ranged from 10 to 47 per year, with the highest incidences in 2004-2006. Gastrointestinal tract manifestations were found in >60% of the cases, potentially confounding diagnosis and leading to inappropriate therapy. Dyspnoea, acute respiratory failure, renal failure, increased serum creatinine and urea levels, increased haematocrits and the presence of pulmonary interstitial infiltrate were significantly more common in HPS patients who died. In addition, we demonstrated that the six cases from the midwest region of the state were associated with Juquitiba virus genotype. The case-fatality rate in this region, 19·2%, was lower than that recorded for other mesoregions. In the multivariate analysis increase of serum creatinine and urea was associated with death by HPS. Our findings help elucidate the epidemiology of HPS in Brazil, where mast seeding of bamboo can trigger rodent population eruptions and subsequent human HPS outbreaks. We also emphasize the need for molecular confirmation of the hantavirus genotype of human cases for a better understanding of the mortality-related factors associated with HPS cases in Brazil.

Pregnant goats were induced to parturition on day 145 of pregnancy, with three different protocols: group Cl (n = 19) was injected intramuscularly (IM) with 75 microg of the prostaglandin analogue R-Cloprostenol; group L (n = 20) was treated IM with 7.5 mg of the prostaglandin analogue Luprostiol; group L(50) (n = 18) was injected IM with 3.75 mg of Luprostiol (IM); in addition, Group S (Control, n = 15) was injected IM with 1 ml of saline solution. Thereafter, goats were continuously observed to record the following parameters: parturition, dystocia incidence, placental delivery and kid and maternal survival. Moreover, blood sampling was performed around kidding and plasma progesterone concentrations were analyzed. The interval from injection to parturition (mean +/- SEM) was not significantly different among the experimental groups: 35.1 +/- 1.5 h, 33.3 +/- 0.9 h and 34.1 +/- 1.8 h (groups Cl, L and L(50), respectively). In the control group, time to parturition was 99.4 +/- 12.1 h (range: 34-166 h). All the goats expelled the foetal membranes within the first 2 h after the induction. The incidence of dystocia due to foetal posture was not significantly different between induced and control goats (21.1%, 20.0%, 22.0% and 20%, for groups Cl, L, L(50) and S, respectively). The percentage of live kids was practically similar between induced goats (93.9%, 94.9% and 92.1%, for groups Cl, L and L(50), respectively); in addition, there was a case of maternal mortality in control group (6.7%; 1/15), whereas there was no mortality in induced goats (0%; 0/57). Plasma concentrations of progesterone showed an intense drop (<2 ng/ml) at 24 h after induction. This study confirms the effectiveness of the luprostiol to induce the parturition in goats, within a narrow range (30-40 h) in most of the induced females (80.0%, 7.5 mg; 77.8%, 3.75 mg).
