Background: Reversible posterior leukoencephalopathy syndrome (RPLS) is a clinicoradiologic syndrome characterized by headache, decreased alertness, seizures, visual abnormalities, and white matter changes indicative of cerebral edema. Although the pathogenesis remains poorly understood, several etiological causes have been described. RPLS is a common complication of chemotherapeutics because of its toxic effect on the central nervous system. This syndrome is frequently associated with seizures but rarely seen with status epilepticus and periodic lateralized epileptiform discharges (PLEDs).
Case Report:We present a case with metastatic lung cancer that developed RPLS after carboplatin and paclitaxel therapy. Our case was admitted to the hospital with status epilepticus and her electroencephalography showed PLEDs. Conclusion: It is important to closely monitor blood pressure and electrolyte levels in patients who take chemotherapeutic agents, especially when there is no previous history of hypertension. It should be kept in mind that RPLS is a causative factor of status epilepticus and PLEDs.
ÖZETÇEİnsan beyninde yaşlanmanın etkisi ile demans gibi oluşan çeşitli hastalıkların tehşisi ve tedavisi, dünyada ve ülkemizde yaşlı nüfusun artması ile günümüz araştırmalarında önemli bir yer tutmaktadır. Hücre ölümlerine bağlı doku kaybı olarak tanımlanan beyin atrofisi, beynin bir kısmını ya da tümünü etkileyen ve demans vakalarında görülebilen bir özelliktir. Atrofi varlığının ve şiddetinin belirlenmesi amacıyla uzmanlar manyetik rezonans görüntülerini kullanarak lateral ventrikül ve sulkuslar gibi beyin bölgelerinde görsel değerlendirmelere başvurmaktadırlar. Bu bağlamda, demansla ilişkili olduğu düşünülen lateral ventrikül ve santral sulkus boyutlarının 3 boyutta otomatik olarak ölçülmesi ve bu ölçümlerin uzmanların atrofi derecelendirmeleriyle uyumunun incelenmesi amaçlanmaktadır. Bu kapsamda, unutkanlık şüphesi ile hastaneye başvurmuş vakaların MR görüntülerinden lateral ventrikül ve santral sulkus yapılarının uzunluk, alan ve hacim gibi değerlerini ölçen ve uzmanlar tarafından oluşturulan atrofi derecelendirmeleri ile kıyaslayan görüntü işleme temelli bir çalışma sunulmuştur. Sunulan yöntem ile elde edilen lateral ventrikül hacim ölçümleri ile atrofi dereceleri arasında yüksek uyum (0.84 civarında) gözlenmiştir. Cerebral atrophy is a feature observed in dementia patients and described as neuronal loss or cell death affecting part or all of the brain, to determine the presence and severity of atrophy, experts visually evaluate magnetic resonance images of the brain, especially at locations such as lateral ventricles and central sulci. The aim of this study is measuring the sizes (e.g. length, area and volume) of lateral ventricles and central sulci in 3D and to compare these measurements with experts' atrophy ratings. Lateral ventricle width shows high agreement (around 0.84) with atrophy grades.
