La atrofia muscular espinal (AME) es una enfermedad neurodegenerativa de herencia autosómica recesiva, que ocasiona degeneración de las motoneuronas del asta anterior medular, con la consecuente debilidad muscular. Los pacientes con AME tipo 1 o enfermedad de Werdnig Hoffmann representan el 50% de todos los tipos de AME. Es la primera causa genética de mortalidad en lactantes, debido principalmente a complicaciones respiratorias. El tratamiento incluye un abordaje multidisciplinar. Tras la aprobación del oligonucleótido antisentido Nusinersen, basado en los resultados de dos estudios pivotales: ENDEAR (AME de inicio temprano) y CHERISH (AME de inicio tardío), que han demostrado mejorías significativas tanto en la supervivencia como en la función motora de los pacientes con AME, se ha producido un cambio de paradigma en el tratamiento de la enfermedad. Además, existen nuevas líneas de investigación con terapia génica, aprobándose recientemente por la Food and Drug Administration de Estados Unidos (FDA), AVXS-101 (Zolgensma®), una nueva terapia de reemplazo génico. Presentamos un caso clínico de una lactante de 7 meses con hipotonía, con clínica y pruebas neurofisiológicas compatibles con atrofia muscular espinal, confirmándose mediante estudio genético, en la que se inició tratamiento con Nusinersen al diagnóstico, apreciándose una ligera mejoría en la función motora y estabilidad desde el punto de vista respiratorio. Nunca antes ha sido tan importante el diagnóstico y tratamiento precoces, ya que los resultados de las nuevas terapias se relacionan directamente con el tratamiento temprano.
La ataxia de Friedreich es una enfermedad neurodegenerativa progresiva de transmisión autosómica recesiva, que conforma la ataxia hereditaria más frecuente en nuestro medio. Se produce por una expansión de más de 33 repeticiones del triplete GAA, secundaria a una mutación del cromosoma 9q13. Se manifiesta antes de los 25 años con síntomas neurológicos, locomotores, endocrinológicos y cardiológicos, siendo estos últimos los que marcan el pronóstico. La esperanza de vida es de 40-50 años. Existen medicamentos que pretenden mejorar los síntomas y detener la progresión de la enfermedad, aunque no disponemos por el momento de ningún tratamiento curativo.Exponemos el caso de un niño de 5 años con un antecedente familiar de atrofia espinocerebelosa, que debutó a los 2-3 años con síntomas neurológicos, presentando en el momento del diagnóstico una miocardiopatía hipertrófica. El paciente fue tratado con idebenona con evolución inicial desfavorable, mejorando posteriormente al aumentar la dosis del fármaco.
