Цель исследования-оценка клинической эффективности и переносимости отечественного толперизона (калмирекс) в сравнении с тизанидином и баклофеном в терапии острой неспецифической скелетно-мышечной боли (НСМБ) у лиц пожилого возраста. Пациенты и методы. Обследовано 135 больных пожилого возраста (60-75 лет) с острой НСМБ. Для купирования болевого синдрома всем пациентам назначали нимесулид в дозе 100 мг 2 раза в сутки после еды в форме суспензии. Пациенты были рандомизированы случайным методом конвертов на три группы, каждая из которых принимала миорелаксант в таблетированной форме в течение 15 дней: группа 1 получала отечественный толперизон (калмирекс) в дозе 150 мг 3 раза в сутки; группа 2-тизанидин в дозе 2 мг 3 раза в сутки; группа 3-баклофен в дозе 10 мг 2 раза в сутки. Оценку болевого синдрома осуществляли по визуальной аналоговой шкале (ВАШ). Обязательными были измерение артериального давления с проведением ортостатической пробы, проверка координаторной системы, регистрация нежелательных явлений (НЯ). Результаты и обсуждение. На фоне лечения во всех группах наблюдался существенный регресс болевого синдрома по ВАШ через 15 дней: в группе 1-с 68 до 14 баллов, в группе 2-с 64 до 17 баллов, в группе 3-с 62 до 18 баллов (без достоверных различий между группами). НЯ чаще встречались у пациентов групп 2 и 3, которые получали тизанидин или баклофен. Ортостатическая гипотензия зарегистрирована у 3 пациентов, принимавших баклофен. Отмечены высокая безопасность и хорошая переносимость комбинации нестероидного противовоспалительного препарата (НПВП) и миорелаксанта в группе пациентов, леченных калмирексом. Хотя миорелаксанты обеспечивают клинически значимое облегчение боли в краткосрочной перспективе при острой неспецифической боли в спине, необходимо учитывать развитие возможных НЯ на фоне их приема у пожилых пациентов, что связано с замедленными метаболическими реакциями, снижением печеночного кровотока и скорости клубочковой фильтрации, а также увеличением числа заболеваний и принимаемых лекарственных препаратов. Заключение. Терапия острой НСМБ у лиц пожилого возраста НПВП и центральными миорелаксантами требует особой осторожности из-за возможного развития НЯ, которые реже наблюдаются при приеме калмирекса, чем при использовании тизанидина и балкофена. Objective: to comparatively evaluate the clinical efficiency of cervicocranialgia therapy with dexketoprofen (flamadex) and tolperisone (calmyrex), as well as with their combination. Patients and methods. The investigation enrolled 90 patients aged 30–60 years with cervicocranialgia, who were randomized into three equal groups. Group 1 included 7 men and 23 women who took dexketoprofen; Group 2 consisted of 13 men and 17 women who were prescribed dexketoprofen and tolperisone; Group 3 comprised 18 men and 12 women who used tolperisone. The three patient groups underwent assessment of the intensity of pain on a visual analogue scale and the degree of muscle tone on a 3-point scale and evaluation of the efficiency of therapy and the hemodynamic effect of the drugs in the common carotid and vertebral arteries. Results and discussion. In all the groups, their treatment reversed neck pain, headache, and dizziness, normalized muscle tone, and improved hemodynamics in the carotid and vertebral arteries. The effect was more pronounced in patients receiving combination treatment (Group 2). The therapy showed a high safety and a good tolerability. Conclusion. Dexketoprofen and tolperisone have been demonstrated to be effective and safe in treating cervicocranialgia.
Objective: to study the clinical efficacy and tolerability of naftidrofuryl (Duzofarm®) in patients with chronic cerebral ischemia (CCI).Patients and methods. A prospective open-label multicenter observational study evaluated the clinical efficiency and tolerability of naftidrofuryl treatment in patients with CCI. To statistically evaluate the efficacy and tolerability of naftidrofuryl, the investigators used data from 200 outpatients with Stages I–II CCI who were included in the program of treatment and received its full cycle. The patients were prescribed naftidrofuryl at a dose of 100 mg (2 tablets) thrice daily. Basic therapy that the patients had received before their inclusion in the observation program was not discontinued. The patients' status was evaluated using a specially designed questionnaire. Four visits were scheduled. All the patients underwent neurological examination. To study the patients' cognitive functions and emotional state, the investigators used the following tests and scales: the Mini-Cog test, the Mini-Mental State Examination (MMSE), the Zung Self-Rating Anxiety Scale, the Asthenia Subjective Assessment Scale, and the Modified Scoring Scale for Subjective Sleep Characteristics. Adverse reactions were recorded to evaluate the tolerability of therapy.Results and discussion. Naftidrofuryl therapy significantly improved health and reversed complaints in the patients. By the end of the treatment cycle, there was a significant improvement on all scales, which suggested a decrease in the severity of cognitive impairment and asthenic and mild anxiety disorders. When performing the Mini-Cog test, the proportion of patients who were able to remember and recall three words without errors increased from 43 to 86%, and when doing the clock drawing test, the proportion increased by 65%. Cognitive functions on the MMSE were observed to statistically significantly improve by 1.2 scores. According to the Zung Anxiety Self-Rating Scale, the number of patients with anxiety disorders decreased by 24%, those with insomnia significantly declined by 31% compared with the baseline level.Conclusion. The findings showed the high efficiency and appropriateness of naftidrofuryl administration to patients with Stages I–II CCI and hypertension.
