en un 0,2% de los individuos tratados (2,3); predomina en la raza blanca pero el riesgo se incrementa hasta 4 veces en los pacientes de raza negra, lo que sugiere que puedan estar implicados factores genéticos o ambientales. Se estima un aumento progresivo en la incidencia del AG secundario a este grupo de fármacos ya que los IECAs cuentan cada vez más con nuevas indicaciones terapéuticas, lo que consecuentemente elevará el número de pacientes en tratamiento con ellos.La aparición de AG no tiene relación con la dosis administrada y en la mayor parte de casos descritos aparecen al inicio del tratamiento, pero hay casos descritos tras 5 años de tratamiento (4) .El AG puede variar desde sólo la tumefacción de la cara, edema de la lengua y del área supraglótica hasta la aparición, más raramente, de edema de las manos, pies y vísceras abdominales.En el mecanismo de aparición del AG se ha involucrado la bradikinina, un nonapéptido potente vasodilatador y proinflamatorio (5). El AG secundario a IECA no se considera una reacción alérgica y en algunos casos puede ser refractario al tratamiento con antihistamínicos y corticoides. (6) En la resolución del AG inducido por IECA es fundamental la suspensión del fármaco, ya que de continuarse puede comprometerse la vía aérea, así como la administración de corticoides y antihistamínicos. Endocarditis nosocomial por Klebsiella oxytocaSr. Director:Las Klebsiella spp se consideran una causa poco habitual de endocarditis infecciosa (EI) y representan el agente etiológico en menos del 1,2% de EI sobre válvula nativa en amplias series revisadas (1). Casi todos los casos corresponden a Klebsiella pneumoniae (1-3) y sólo hemos encontrado publicados 3 casos debidos a Klebsiella oxytoca (4-6) por lo que consideramos de interés describir el caso que referimos a continuación.Se trata de un paciente de 75 años con antecedentes de alergia comprobada a penicilina, derivados y cefalosporinas. Entre sus antecedentes patológicos destacan hipertensión arterial, hipercolesterolemia, insuficiencia renal crónica, y RTU de próstata por hiperplasia benigna. Se le diagnosticó de forma incidental por ecografía abdominal un aneurisma de aorta abdominal (AAA) infrarrenal de 45 mm y se confirmaron los hallazgos con angio-RMN. Fue intervenido de forma programada y se le colocó una endoprótesis aortoilíaca izda y by-pass femoro-femoral con buena evolución clínica inicial. Trece días después comenzó a presentar picos febriles a diario con Tª > 39º. En el cultivo de la herida quirúrgica, cultivo del drenaje y urocultivo se aisló Staphylococcus epidermidis e inicialmente fue tratado con ciprofloxacino, gentamicina y teicoplanina a dosis ajustadas sin respuesta clínica. En 2 hemocultivos se aisló Klebsiella oxytoca sensible a cefalosporinas, imipenem, gentamicina y ciprofloxacino. Se realizaron TC abdominal y gammagrafía con leucocitos marcados en los que no se comprobaron signos de infección protésica ni otros hallazgos reseñables y en la Rx de tórax no se apreció neumonía.
