Tracheobronchopathia osteochondroplastica (TO) is a rare chronic disease of the airways characterized by the growth of cartilage and/or bone tissue in the submucosal layer of the trachea and large bronchi with varying degrees of their lumen narrowing. In a significant percentage of cases diagnosis is made only posthumously due to the late onset of clinical manifestations and the considerable complexity of life-time diagnostics.The objective of the research was to conduct the clinical observation of OT.Materials and methods. A patient Sh., 49 years of age, was admitted to the department of differential diagnostics of regional clinical pulmonology center to determine the cause of prolonged cough and bronchial obstruction syndrome. Only fiber-optic bronchoscopy (FBS) was the only informative method of examination. On the mucous membrane of the trachea and right main bronchus the dense white protuberance was found. The biopsy material was taken. Histological examination revealed areas of calcified cartilage located in subepithelial layer. Results. Taking into account the reasearch data, the TO was suspected. The disease has a benign course for many years and is asymptomatic at initial stages. However, deformation and narrowing of the lumen of the trachea and the large bronchi with epithelial atrophy causes a disturbance of phlegm discharge. This promotes the development of infectious and inflammatory processes in the lung tissue. Patients complain of cough, hemoptysis, shortness of breath, chest pain. X-ray examination of the lungs may detect the infiltrative changes. The indicators of lung function are normal or correspond to the type of obstructive disorders. CT radiograph and virtual bronchoscopy determine the deformation and narrowing of the trachea and the large bronchi due to the elements of cartilage and bone tissue. Definitive diagnosis is possible only according to the results of FBS and histological examination.Conclusions. Awareness on such rare pathology as TO is essential, especially in patients with chronic cough. In case of absence of specific clinical and radiographic changes, the computed tomography of the chest and FBS with for histological examination sampling should be performed to confirm the diagnosis.
Резюме. У роботі розглянуто структурні особливості міокарда щурів із інсулінорезистентністю (ІР), які перебували на стандартному або йододефіцитному харчових раціонах. Установлено, що у тварин із ІР частина міоцитів міоцитів зменшені в поперечнику, окремі капіляри у просвіті містили нагромадження еритроцитів та лейкоцитів. У артеріолах і дрібних артеріях, окремих капілярах відзначали ендотеліальну гіперклітинність. Стінка частини таких судин нерівномірно гомогенна, з ділянками просвітлень. Більш суттєві зміни структурної організації міокарда виявлено у щурів із ІР, обтяженою йододефіцитом. Виявлено, що в частині випадків пухка сполучна тканина поміж кардіоміоцитами розширена за рахунок набряку (паренхіматозно-інтерстиційний індекс менший на 83 %, р<0,001 щодо контролю). У більшості кардіоміоцитів простежується зниження вмісту глікогену, а в окремих клітинах не відзначається зовсім. Паренхіматозні клітини міокарда із завуальованими межами, цитоплазма просвітлена та гранулярна, нечітко верифікується поперечна посмугованість.
Резюме. У щурів із інсулінорезистентністю (ІР) виявлено розвиток регенераторних процесів у клітинах печінки, зокрема в ядрах, на що вказує збільшення ядерно-цитоплазматичного індексу та наявність двохядерних гепатоцитів. Проте напруженість регенераторного процесу є невисокою (встановлено зменшення співвідношення площі ядерця до площі ядра гепатоцитів). Статевий диморфізм полягає у наявності осередків жирової дистрофії та вираженому зональному характері дистрофічних процесів гепатоцитів у самок порівняно з самцями, у яких дистрофія має більше дифузний характер. Обтяжує розвиток структурних змін у печінці йододефіцит, особливо, вроджений. Гепатобіліарний ризик за умов ІР на тлі йодної депривації вищий у самок. Relevance. The relationship between endocrinopathies and the condition of the oral cavity is associated with disorders of metabolism, hemodynamics, immunological and neuroregulatory changes. Since the first line of contact with the environment of the oral cavity is the mucous membrane, changes in its properties adversely affect the functional state of other periodontal tissues.
Objective: to examine the structural changes of the oral mucosa in rats with combined endocrinopathy.
Materials and methods. The studies were carried on 120 rats, which were divided into three experimental (animals with iodine deficiency – ID, insulin resistance – IR, IR on the background of ID) and a control group of animals. Histological examination of the oral mucosa, computer morphometry, and densitometry of objects was performed.
Results. In rats with ID, the epithelium of the mucous membrane was characterized by the development of acanthosis, expansion of the granular layer, enhanced keratin formation. The development of IR caused hyperplastic changes in the epithelium with the formation of unexpressed cords of acanthosis, epithelial basal hypercellular infiltration, which was confirmed by morphometric data. Under conditions of combined endocrinopathy, more pronounced changes in the histological structure of oral mucosa were observed. In particular, the development of mucosal edema in connective tissue, with predominantly macrophage hypercellularity at the periphery, was noted. Densitometric examination of the samples indicated a decrease in the optical density of connective tissue. At the same time, hyperplastic changes in most layers of the epithelium and pronounced changes in the vascular wall were observed.
Conclusions. Combined endocrinopathy leads to pronounced changes of the oral mucosa, which can lead to disruption of the structure of periodontal tissues and the entire tooth-alveolar complex.
