ne meilleure intégration des pratiques cliniques préventives (PCP)* dans la pratique médicale permettrait de réduire la morbidité et la mortalité liées à certaines maladies évitables 1. Pour soutenir cette intégration, des guides de PCP ont été développés par le Groupe d'étude canadien sur les soins de santé préventifs (GECSSP) et par l'U.S. Preventive Services Task Force (USPSTF). Cependant, plusieurs études indiquent que l'intégration souhaitée demeure encore difficile. Parmi les barrières, le manque de temps est souvent identifié comme étant un obstacle majeur, auquel s'ajoutent d'autres facteurs propres au clinicien, à l'environnement de pratique ou aux PCP elles-mêmes 2-6. Par rapport aux adultes américains qui reçoivent en moyenne 55 % des soins préventifs reliés aux principales maladies, causes de mortalité et de consultation 7 , les médecins généralistes de l'Ontario appliquent à leurs patients dans une proportion de 65 % les recommandations de catégorie A du GECSSP et seulement dans 31 % celles de catégorie B 8. En Colombie-Britannique, les médecins de famille réalisent le dépistage du cancer du sein et du col utérin ainsi que le counselling anti-tabac dans 90 % des cas 9. Au Québec, les habitudes de vie sont abordées par les généralistes dans seulement 10 à 37 % des visites médicales, en particulier chez des patients déjà atteints de maladies chroniques ou de facteurs de risque associés 10. Pour les médecins spécialistes, peu de données sont disponibles. Cette étude visait à décrire, chez des médecins oeuvrant au Centre hospitalier de l'Université de Montréal (CHUM), le portrait de l'intégration des PCP de catégorie A et B du GECSSP et de l'USPSTF. L'étude avait aussi pour objectif d'identifier les barrières rencontrées par les médecins pour leur mise en oeuvre et de décrire leur pratique personnelle de saines habitudes de vie ainsi que leurs attitudes face à la prévention. MÉTHODE Population à l'étude L'étude a été réalisée au cours des mois de juin-juillet 2006 auprès de tous les médecins exerçant dans les départements de biochimie, chirurgie, médecine, médecine générale, obstétrique-gynécologie,
El presente trabajo presenta un panorama del acceso de la población con discapacidad a la educación superior y de su terminalidad. Se basa en estudios antecedentes de América Latina y fuentes secundarias de las estadísticas referentes a Argentina y a la provincia de Córdoba. En estos dos últimos contextos, se profundiza sobre el alcance comparativo de estudios superiores en población con y sin discapacidad y datos desagregados por cada tipo de discapacidad. Los resultados visibilizan la desigualdad, aún existente, de acceso y terminalidad en la educación superior mediante el análisis de la interrelación entre la condición de discapacidad de cada sujeto y las oportunidades sociales que ofrece este nivel educativo. Se plantean hipótesis e interrogantes sobre la relación que estos hallazgos pueden tener con las medidas de accesibilidad más y menos consolidadas en las instituciones de educación superior.
There is a growing interest in vertically transmitted viruses. Field studies show that 15 virus infection frequency often varies in space and time. In order to understand the 16 spatial variation of virus prevalence, we designed a mathematical model describing 17 the infection dynamics of a biparentally inherited virus in a two-population model. 18Model parameters are paternal and maternal transmission rates, cost of infection, 19 and up to two migration rates. We investigated three different population structures: 20(1) a single panmictic host population, (2) two populations connected by unidirec-21 tional migration (mainland-island model), and (3) two populations connected by 22 two-way migration. According to the results, the parameter space can be divided in 23 three zones: (1) viral spread, (2) no viral spread, (3) bistability (in a single popula-24 tion) or stable infection polymorphism (in the two-population models). The finding 25 of bistability is interesting and new. It shows that infected and uninfected popu-26 lations can stably coexist, if migration is below a critical value, and if the cost of 27 infection is moderately high. In summary, our results show that spatial structure of 28 host populations is an important factor in determining geographical differences of 29 vertically transmitted viruses.30 31
