INTRODUCTION : La tuberculose splénique est l’un des aspects rares de la tuberculose hématopoïétique des organes profonds. Le but de notre communication est de souligner, en rapportant un cas de tuberculose splénique pseudo-tumorale, d’une part son polymorphisme clinique et anatomique, et d’autre part le rôle diagnostique et thérapeutique de la chirurgie. OBSERVATION : Il s’agissait d’une patiente de 24 ans qui présentait une splénomégalie importante accompagnée de fièvre et d’amaigrissement. L’échographie et la TDM abdominopelvienne montrait des images d’une rate polykystiques isolée évoquant un abcès splénique. Une splénectomie a été réalisée et l’analyse bactériologique a montré la présence de Mycobacterium tuberculosis et l’analyse de la pièce opératoire a confirmé le diagnostic de tuberculose splénique. Les suites opératoires ont été simples et la patiente a été mise sous traitement anti-bacillaire. CONCLUSION : La tuberculose splénique est une entité rare dans notre contexte, qui doit être évoquée devant toute splénomégalie isolée avec notion de fièvre au long cours. Le diagnostic de certitude est histologique et/ou bactériologique. Le traitement repose sur une polychimiothérapie anti-bacillaire, qui si elle est débutée précocement permet la guérison.
INTRODUCTION : Les cancers sont des maladies chroniques qui deviennent de plus en plus fréquentes dans les pays du Sud du Sahara. Leur prise en charge est complexe. Cela nécessite une réunion d’équipe pluridisciplinaire avant toute prise de décision thérapeutique. Le but de notre communication est d’évaluer l’impact de la réunion de concertation pluridisciplinaire dans la prise en charge chirurgicale des cancers dans notre unité. MATERIELS ET METHODES : Il s’agissait d’une étude observationnelle rétrospective portant sur les dossiers des patients discutés à la RCP du 15 Novembre 2017 au 31 Mars 2019. RESULTATS : Quarante-et-six séances de RCP avaient été organisées. Les dossiers de 222 patients ont été présentés avec une décision de chirurgie pour 100 (45,0%). On notait une prédominance féminine avec un âge moyen de 41,8 ± 18,3 ans. Soixante-et-neuf pour cent des patients avaient une tumeur maligne. Soixante-et-un patients ont été opéré conformément à la décision de la RCP et deux avec changement de décision. Les marges de résection tumorale étaient saines pour trois patients sur quatre. Le délai médian de prise en charge entre la RCP et l’intervention chirurgicale était de 14 jours (extrêmes de sept à 34). CONCLUSION : La RCP constitue une avancée dans la prise en charge des patients à l’UCO. Une réévaluation régulière améliorera la prise en charge des patients.
