Розглянуті питання класифікації і диференціальної діагностики езофагітів у дітей шкільного віку. Описані найбільш характерні симптоми езофагітів різної етіології. Матеріали та методи. Обстежено 60 дітей шкільного віку, у яких визначалась характерна для езофагіту клінічна симптоматика. Діагноз верифікований ендоскопічно та морфологічно. Результати. За даними ендоскопічного дослідження в усіх дітей були виявлені вогнищева або дифузна еритема та набряк слизової оболонки, у 53,3 % дітей-множинні білуваті нальоти, у 40 % дітей-лінійні ерозії. При морфологічному дослідженні найпоширенішими змінами були: гіперемія судин мікроциркуляторного русла-у 85 % випадків, вакуольна дистрофія епітеліоцитів-у 76,7 % випадків, діапедезні крововиливи-у 63,3 % випадків, деструкція гранул кератогіаліну-у 50 % випадків. Висновки. Для вирішення проблеми діагностики хронічних езофагітів у дітей необхідний підхід з використанням усієї сукупності сучасних методів діагностики, які зможуть допомогти верифікувати правильний діагноз та визначитися з напрямками подальшого лікування. Морфологічне дослідження є обов'язковим та вирішальним при встановленні діагнозу езофагіту.
Вступ Харчова алергія (ХА)-це побічна реакція на їжу, зумовлена імунологічними механізмами, тому цей термін охоплює IgE-опосередковані, не-IgE-опосередковані й змішані (IgE-опосередковані й не-IgE-опосередковані) реакції. Алергічні реакції можуть виникати на будь-які харчові продукти, але найчастіше-на коров'яче молоко, куряче яйце, пшеницю, сою, арахіс, лісові горіхи, рибу й морепродукти [1]. За даними European Academy of Allergy and Clinical Immunology [2], у середньому в європейських країнах 17,3 % населення мають ті чи інші прояви ХА протягом життя. При цьому підвищена концентрація специфічних IgE до харчових продуктів визначається у 12,2 % дітей і підлітків (віком до 18 років) і 4,1 % дорослих [3, 4]. The article summarizes the data on atopic dermatitis problem in pediatric practice, in particular, the issues of its diagnosis and treatment are considered. Atopic dermatitis is the most common skin disease in children, which greatly reduces small patients quality of life. It is well known that this disease is the result of the complex interaction of defects in the barrier function of skin, immune dysregulation, and environmental and infectious agents. There are no specific diagnostic tests for atopic dermatitis. There is no pathognomonic laboratory biomarker for its diagnosis. One of the earliest and most recognized diagnostic criteria is the Hanifin and Rajka diagnostic criteria of 1980. The diagnosis is performed in the presence of three main criteria and three additional criteria. Approaches to its treatment and choice of effective and safe treatment method in childhood are particularly relevant in recent years. For children with atopic dermatitis, a differentiated, integrated approach to treatment is necessary. Successful treatment requires a multifaceted approach that involves teaching of the patient and care-takers, elimination diet in the event of a food allergy, optimal skin care, anti-inflammatory therapy with topical corticosteroids and/or topical calcineurin inhibitors, and treatment of skin infections. The main part of treatment is daily proper skin care. The use of moisturizers enhances the hydration of the skin. Emollients can reduce the inflammation and the severity of the disease. In addition, their use reduces the amount of anti-infectious agents to fight the disease.
Резюме. Здоров'я і харчування-це сфера найбільш актуальних досліджень на сьогодні. Часто буває, що страви, які приносять нам задоволення, позбавляють нас деяких потрібних поживних речовин, що призводить до виникнення різних недуг, таких як гіперглікемія, гіперхолестеринемія, захворювання кишечника тощо. За останні кілька десятиліть зріс попит на функціональні продукти харчування. Кілька досліджень показали, що інулін функціонує як пребіотик, сприяючи доброму травленню, а також впливає на ліпідний обмін і зменшує ризик остеопорозу за рахунок збільшення абсорбції кальцію. Встановлено, що інулін має вплив на інсулінемію і глікемію, а також зменшує ризик раку товстої кишки. Останні дослідження показали, що він сприяє біосинтезу прекурсора вітаміну D.
Антибіотики -це лікарські препарати, що часто використовуються в педіатричній практиці. Їх застосування пов'язане з різноманітними побічними ефектами. Найпоширенішими з них є шлунково-кишкові, такі як нудота й діарея. Антибіотикоасоційована діарея (ААД) виникає в тих випадках, коли антибіотик порушує екологію мікробіоти кишечника, змінюючи різноманітність і кількість бактерій у ньому. Ці зміни можуть вплинути на здатність резидентної мікробіоти протистояти вторгненню патогенних мікроорганізмів [1] або зростанню умовно-патогенних видів, що ен-догенно присутні в мікробіомі [2,3]. Навіть після відновлення загальної кількості бактерій може спостерігатися тривалий вплив на баланс кишкової мікробіоти і, отже, на сприйнятливість пацієнта до інфекції та інших захворювань [4,5]. Тому AAД може призвести до тривалої госпіталізації, збільшення витрат, погіршення якості життя та інших ускладнень. Діарея найчастіше пов'язана із застосуванням антибіотиків широкого спектра дії [6][7][8][9].У педіатричній популяції захворюваність на ААД становить 5-30 %, у дорослій -5-70 % [10].
