HintergrundSeit fast 90 Jahren werden Mikroskope im Rahmen medizinischer Therapieverfahren eingesetzt. In den 50er-Jahren des letzten Jahrhunderts brachte die Firma Carl Zeiss das erste kommerziell erhältliche binokulare Operationsmikroskop zum Einsatz, welches zunächst in der Ohrenheilkunde zur Anwendung kam. Bis zur Nutzung von Operationsmikroskopen in der Zahnmedizin vergingen weitere 30 Jahre. Apotheker und Jako beschrieben 1981 erstmals die Entwicklung und den Einsatz eines Operationsmikroskops im Rahmen der zahnärztlichen Therapie [1]. Der dentalen Anwendung wurde dann von Carr und Shanelec zu Beginn der 90er-Jahre zum Durchbruch verholfen. Bestimmte Veränderun-gen des herkömmlichen Operationsmikroskops führten zur Entwicklung des heute bekannten dentalen Operationsmikroskops [7], (Abb. 1). Inzwischen sind die verschiedensten oralen Anwendungen bekannt, unter anderem der Einsatz im Rahmen mikrochirurgischer Operationstechniken in der Parodontologie, aber vor allem auch zur Optimierung der endodontologischen Diagnostik und Therapie. Vorteile des Operationsmikroskops in der EndodontieDer Erfolg endodontischer Maßnahmen wird nicht zuletzt von einer guten Sicht auf das Operationsfeld und der nachfolgend erfolgreichen Therapiemaßnahmen bestimmt. Deshalb ist es verständlich, dass der Einsatz des dentalen Operationsmikroskops im Vergleich zum "unbewaffneten" Auge zu deutlichen Vorteilen in der endodontologischen Diagnostik und Therapie führt. Durch die starke Vergrößerung und optimale Ausleuchtung des Arbeitsfeldes wird ein präzise-res Arbeiten möglich. Mit einer bis zu 25-fachen Vergrößerung der Strukturen lassen sich auch kleinste anatomische Besonderheiten darstellen. Durch einen fest definierten Abstand von 20-30 cm zwischen Objekt und Mikroskop ist auch das ergonomische Arbeiten des praktischen Zahnarztes beziehungsweise Endodontologen gewährleistet. Bei richtiger Patientenlagerung und Arbeitshaltung kann man entspannt und gerade sitzen und insgesamt die Anstrengung minimieren (Abb. 2). Die durch das Mikroskop verbesserte Sicht auf das Arbeitsfeld ermöglicht ein genaueres und grazileres Arbeiten. Es wird speziell auch das Risiko des unnötigen Verbiegens der Wurzelkanalinstrumente insbesondere bei der Kanaleingangssuche reduziert. Außerdem sinkt das Frakturrisiko der Instrumente und man kann davon ausgehen, dass es auch deutlich seltener zu einer Via falsa kommt [7].
