Mitteilungen über schwere, durch Operation und Sektion -meistens durch beides -gesicherte Pankreasnekrosen während der Gravidität oder nach dem Partus reichen in das vorige Jahrhundert zurück. Noch bis in die jüngste Zeit sind Einzelbeobachtungen beschrieben. Im deutschen Schrifttum hat Marcus (21) die ihm bis 1930 erreichbaren 44 Veröffentlichungen zusammengetragen. Seitdem sind uns bis 1959 weitere 32 Arbeiten, zum Teil über mehrere Kranke, bekanntgeworden (Literatur bei Gülzow [10]). Einige aus jüngster Zeit kommen hinzu, so daß die Gesamtzahl der kasuistischen Publikationen 100 kaum überschreiten dürfte. Dieser Uberblick gibt kein wirkliches Bild von der Häufigkeit der Gestationspankreatitis, da nur besondere und schwere Erkrankungen bekanntgegeben wurden. Die weit größere Zahl leichterer Pankreatitiden dürfte nicht erfaßt sein. Die Pankreatitis kommt im gebärfähigen Lebensalter -nach Jacobs und Sanders vom 17. bis 44. Lebensjahr -bei Erst-und Mehrgebäreriden in jeder Phas.e der Gravidität. häufiger jedoch in der zweiten Hälfte und post partum vor. Langmade und Edmondson (16), die 53 Kasuistiken überprüften, fanden unter den Erkrankten 23 Erst-und 30 Mehrgebärende; bei 39 Frauen trat die Pankreatitis post partum auf. Marcus (21) erwähnt Mitteilungen über Frauen, die im Verlauf mehrerer Schwangerschaften Pankreatitisschübe hatten: Eine Patientin von Galliard und Chifoliau erkrankte in drei aufeinanderfolgenden Sdiwangersthaften an einer Pankreatitis und starb daran in der dritten Gravidität. Prochownik (26) beschreibt eine Mehrgebärende, die in drei Schwangerschaften an Abdominalattacken litt, deren Heftigkeit schließlich eine Operation veranlaßte. Es fand sich-eine Pankreasnekrose, an der die Kranke zugrunde ging. Auch je eine Beobachtung von Körte (14) und Pearce-Gould (25) lassen vermuten, daß sich die Pankreatitis in zwei Schwangerschaften wiederholte. Nach Madlener (18) wurde die Diagnose bei den von Langmade und Edmondson gesammelten 53 Kranken nur dreimal präoperativ gestellt, bei zwei Drittel während der Operation und bei dem Rest durch die Autopsie. Daß die Pankreatitis bei Schwangeren häufiger auftritt als im Durchschnitt der Bevölkerung, ist nicht erwiesen. Sicher ist sie nicht seltener; leichte Schübe werden wahrscheinlich oft fehlgedeutet. Es kommen alle Schweregrade der Erkrankung in der Gravidität und post partum vor; die Bedeutung der schweren Pankreatitiden liegt in der Bedrohung des mütterlichen und kindlichen Lebens. Eigene Beobachtungen Wir behandelten in den letzten Jahren acht Frauen, von denen einige Angaben aus der Tabelle 1 zu entnehmen sind. Uber vier von ihnen (1, 4, 5, 6 der Tabelle) sowie über eine Patientin mit zyklusgebundenen Pankreatitisschüben, diewährend der ersten Schwangerschaft sistierten und post partum wiederkehrten (9), wurde in einer früheren Mitteilung berichtet (10). Bei einer der vier Patientinnen trat der erste schwere Pankreatitisschub im letzten Monat der Gravidität, bei den übrigen drei innerhalb der ersten 15 Tage post partum auf.
