In search of an efficient but simple, low cost procedure for the serodiagnosis of Toxoplasmosis, especially suited for routine laboratories facing technical and budget limitations as in less developed countries, the diagnostic capability of Hematoxo® , an hemagglutination test for toxoplasmosis, was evaluated in relation to a battery of tests including IgG- and IgM-immunofluorescence tests, hemagglutination and an IgM-capture enzymatic assay. Detecting a little as 5 I.U. of IgG antitoxoplasma antibodies, Hematoxo® showed a straight agreement as to reactivity and non-reactivity for the 443 non-reactive and the 387 reactive serum samples, included in this study. In 23 cases presenting a serological pattern of acute toxoplasmosis and showing IgM antibodies, Hematoxo® could detect IgM antibodies in 18, indicated by negativation or a significant decrease in titers as a result of treating samples with 2-mercapto-ethanol. However, a neat increase in sensitivity for IgM specific antibodies could be achieved by previously removing IgG from the sample, as demonstrated in a series of acute toxoplasmosis sera. A simple procedure was developed for this purpose, by reconstituting a lyophilized suspension of Protein A - rich Staphylococcus with the lowest serum dilution to be tested. Of low cost and easy to perform, Hematoxo® affords not only a practical qualitative procedure for screening reactors and non-reactors, as in prenatal services, but also quantitative assays that permit to titrate antibodies as well as to identify IgM antibodies.
No diagnóstico sorológico da doença de Chagas podem ser encontradas acentuadas divergências mesmo entre laboratórios de grande experiência. Para a padronização de um teste imunoenzimático destinado primariamente à seleção de doadores de sangue foram cuidadosamente escolhidos painéis de soros que se buscaram como representativos das populações de chagásicos e de não chagásicos. Produzido para máxima sensibilidade e estabilidade, o novo reagente (bioELISA - cruzi<A NAME="tx01"></A><A HREF="#nt">*</A>) foi testado em 1648 soros, com diagnóstico clínico de doença de Chagas em 219 e de outras afecções em 104. O teste foi comparado com testes já bem padronizados, de imunofluorescência (IF) e de hemaglutinação (HA), em 1325 soros. O limiar de reatividade (cut off), estabelecido como ideal, foi indicado nos testes pela absorbância de um soro de reatividade mínima. A sensibilidade do teste imunoenzimático foi de 0,9954 e a especificidade, como conegatividade, de 0,9969. Não foram vistos resultados falso-positivos em casos de sífilis, toxoplasmose, mononucleose e de soros com altos títulos de anti-estreptolisina 0, mas foram encontrados em 5 de 15 casos de leishmaniose tegumentar, 10 de 24 casos de calazar, 1 de 15 casos de artrite reumatóide e 1 de 12 casos de lupus eritematoso sistêmico. Os altos índices de sensibilidade em chagásicos e de especificidade na população geral, traduzem a elevada confiabilidade do teste para triagem de doadores de sangue e para a confirmação de suspeita clínica de doença de Chagas.

Hemolysis is a challenging problem and still represents a frequent source of errors in blood test laboratory practice. Due to the broad and heterogeneous bias induced in the measurement of several parameters by hemolysis, inaccurate results may be reported, and the patient may be required to repeat sample collection, delaying diagnosis. Existing automated laboratory devices including hemolysis detection are not suitable for lower volume and smaller sample collection sites. In many situations, hemolysis is still detected by visual inspection of the sample after centrifugation, during the blood test pre-analytical stage. Visual inspection is highly dependent on a qualified workforce, subjective to interpretation discrepancies, and thus difficult to standardize. The paper aims to describe the design and performance of a portable device for measuring hemolyzed samples based on computer vision and neural network. The results indicate that the device provides hemolysis indexes with sufficient accuracy to guide laboratory decision in the blood test pre-analytical stage.
