Eine Hilfe zur Abschätzung des zu erwartenden Ergebnisses und zeitlichen Ablaufes unter Berücksichtigung unterschiedlicher PathogenesenOriginalarbeit Eine schmerzhafte Bewegungseinschrän-kung der Schulter bedeutet für die betroffenen Patienten eine deutliche Belastung und Einschränkung in ihrem privaten Umfeld und im Arbeitsalltag. Die Ursachen dieser Erkrankung können vielfäl-tig sein, ohne dass ein genauer Pathomechanismus bekannt ist. Neben posttraumatischen oder -operativen Folgen [8,25,29] wird ein Zusammenhang mit metabolischen Erkrankungen (Diabetes mellitus, Schilddrüsenerkrankungen, Fibroblastenproliferation etc. [15,30]) und eine idiopathische Form diskutiert. Ähnlichkeiten bestehen einigen Autoren zufolge auch zum Krankheitsbild des M. Sudeck [1]. Als Synonym wird der Begriff der "frozen shoulder" und der adhäsiven Kapsulitis, insbesondere im inflammatorischen Zustand, verwandt. Der Versuch einer Stadieneinteilung konnte sich, außer für die idiopathische Form, im Klinik-und Praxisalltag nicht durchsetzen. Zu uneinheitlich zeigt sich das Bild dieses Leidens mit nur wenigen, objektiv fassbaren, Merkmalen wie z. B. Bewegungsausmaß, Kapselverdickung in der Magnetresonanztomographie (MRT), um eine sinnvolle Einteilung als Leitfaden zur Therapie aufstellen zu können [13].Konservative Maßnahmen (Physiotherapie, Analgetikagabe, Entspannungsübungen, Kortisoninjektionen oder Infiltrationen) stehen bei der Behandlung im Vordergrund. Unter diesen Optionen wird in der Mehrzahl der Fälle eine deutliche Besserung mit Wiederherstellung der Beweglichkeit und Schmerzfreiheit erreicht [26,27]. Jedoch bleiben etwa 10% der Patienten im Verlauf therapierefraktär [7]. Für dieses Patientenkollektiv galten lange Zeit die Narkosemobilisation oder die offene Arthrolyse (Bridenlösung, Sehnenplastik des M. subscapularis, Akromioplastik) als Mittel der Wahl. Erst mit der breiten Anwendung arthroskopischer Verfahren an der Schulter konnte eine schonendere Therapie angewandt werden, deren Nutzen im Hinblick auf Schmerzreduktion und Wiederherstellung der Beweglichkeit mannigfaltig nachgewiesen werden konnte [5,6,8,9,12,17,21,28,29]. Des Weiteren zeigen arthroskopische Arthrolysen aufgrund einer besseren Visualisierbarkeit der Pathologien, mit der Möglichkeit einer gleichzeitigen und zielgerichteten Therapie, eine geringere Komplikationsrate (Rotatorenmanschettenrupturen, Plexusläsionen, Frakturen, Instabilität) im Vergleich zu offenem Vorgehen oder einer Narkosemobilisation [6,8,14,21]. Das Hauptaugenmerk bisheriger wissenschaftlicher Arbeiten lag zum einen auf der Darstellung des arthroskopischen Vorgehens und dem Beweis seines Nutzens als effizientes und komplikationsarmes Verfahren [2,28,29].
