Introduction Coronavirus disease 2019 (COVID-19) represents a serious threat to patients on maintenance dialysis. The clinical setting, mortality rate, and prognostic factors in these patients have not been well established. Methods We included all dialyzed patients with COVID-19 referred to our dialysis center between March 11 and April 11, 2020. Data were obtained through the review of the medical records and were censored at the time of data cutoff, on May 11, 2020. Results Forty-four patients on maintenance dialysis with COVID-19 were referred to our dialysis unit during the COVID-19 epidemic. Median age was 61 years (interquartile range [IQR]: 51.5–72.5); 65.9% were men. Comorbidities included hypertension (97.7%), diabetes mellitus (50%), and chronic cardiac (38.6%) and respiratory (27.3%) diseases. Initial symptoms were fever (79.5%), shortness of breath (29.5%), cough (43.2%), and diarrhea (13.6%). Three profiles of severity were distinguished based on the World Health Organization (WHO) progression scale. Forty-one (93.2%) were hospitalized and only 3 were maintained on outpatient hemodialysis. Thirty-three (75%) patients required oxygen therapy, including 15 (45.5%) who were referred to the intensive care unit. Overall, 27.3% of patients died, and 58.5% were discharged from hospital, including only 2 (13.3%) of those admitted to the intensive care unit. By multivariate analysis, cough, thrombopenia <120 g/l, lactate dehydrogenase (LDH) level greater than 2 times the upper limit of normal, and blood C-reactive protein (CRP) >175 mg/l were significantly associated with death. Conclusion A major outbreak of COVID-19 occurred in the Paris region, and spread among dialyzed patients. Our study underscores the severity of COVID-19 in these patients and identified prognostic markers.
La ponction biopsie rénale (PBR) est une technique indispensable au diagnostic des maladies rénales. Ce geste demeure invasif avec diverses complications, essentiellement hémorragiques. L'objectif de notre travail était d'étudier notre pratique de la PBR, mettre le point sur la technique de la biopsie rénale transcutanée, évaluer ses complications, et déterminer les néphropathies diagnostiquées dans notre région. Il s'agit d'une étude rétrospective, menée entre Janvier 2015 et Juin 2017. Nous avons inclus 69 patients du service de Néphrologie du CHU Mohamed VI, Oujda, Maroc ayant bénéficié d'une biopsie de reins natifs. L'âge moyen des patients au moment de la PBR était de 38,16 ±13 ans (12-77 ans), le sexe ratio (H/F) était de 0,86. Le syndrome néphrotique était l'indication la plus fréquente tout âge confondu. Les néphropathies glomérulaires représentaient 81% des maladies rénales, réparties comme suit: la glomérulonéphrite extra-membraneuse retrouvée chez 19% des patients, la hyalinose segmentaire et focale chez 18%, la glomérulonéphrite lupique chez 11%, les glomérulonéphrites chroniques diagnostiquées dans 11% des cas, la glomérulonéphrite membrano-proliférative chez 7%, la glomérulonéphrite extra-capillaire chez 5% à part égale avec l'amylose rénale. Quatre pour cent (4%) des PBR étaient en faveur d'une néphropathie diabétique. Avec un taux estimé à 2% chacune, l'étude histopathologique avait retrouvé la glomérulonéphrite aigue post-infectieuse, la lésion glomérulaire minime, la néphropathie à IgA, les vascularites, les néphropathies tubulo-interstitielles ainsi que la microangiopathie thrombotique. Cinq pour cent (5%) des PBR étaient aglomérulaires. L'hématurie macroscopique était la principale complication observée dans notre série avec une fréquence de 2,8%. La PBR reste le gold standard du diagnostic de la maladie rénale, cependant le diagnostic syndromique permet au clinicien d'identifier la maladie rénale la plus probable et de guider les thérapeutiques urgentes.
La mauvaise observance thérapeutique (OT) est fréquemment rencontrée dans la plupart des maladies chroniques. Elle accroît la morbi-mortalité et les dépenses de santé. Les patients hémodialysés chroniques ont souvent une OT peu satisfaisante. L'objectif de notre étude était d'évaluer le niveau d'OT chez nos patients hémodialysés chroniques et d'identifier les facteurs prédictifs de mauvaise compliance. Nous avons réalisé une étude transversale en novembre 2011 au centre hospitalier d'Oujda. L'OT fût mesurée à l'aide d'un questionnaire: test d'évaluation de l'observance (TEO) tenant compte à la fois de l'adhésion au traitement médicamenteux et au régime alimentaire. Un taux de 85% était retenu arbitrairement comme seuil de bonne OT. Un taux entre 57% et 85% permet de situer le patient dans une zone d'observance partielle, et un taux inférieur à 57% indique la mauvaise OT. Les différents paramètres démographiques et clinico-biologiques ont été analysés et nous avons dégagé les facteurs prédictifs de bonne et mauvaise OT. Notre étude a porté sur 101 patients hémodialysés chroniques, le sexe ratio est de 1,2. La moyenne d'âge 49,7 ± 15,6. Le TEO a objectivé 23,4% de bons observants, 39,4% d'observance partielle et 37,2% de mauvais observants. L'analyse statistique a montré que la mauvaise OT est associée au bas niveau socio-économique et intellectuel et la présence de comorbidités et la longue durée d'hémodialyse. La bonne OT a été observée chez les patients d'âges extrêmes aidés par une tierce personne, le nombre réduit de médicaments et le niveau intellectuel.
