Bildgestützte Chirurgie oder image guided surgery (IGS) bedeutet, dass die prä-oder intraoperativen Abbildungen des operativen Situs und der angrenzenden Organe mit dem chirurgischen Instrument mit einem hohen Präzisionsgrad und in Echtzeit korreliert werden. IGS kann bis zu einem gewissen Grade als eine Erweiterung der minimal-invasiven Chirurgie verstanden werden, die seit den 90er-Jahren des letzten Jahrhunderts eine weite Verbreitung fand. Ebenso wie IGS hat die minimal-invasive Chirurgie zum Ziel, durch einen möglichst präzisen kleinen Zugang den Kollateralschaden auf angrenzende Gewebsschichten auf ein Minimum zu begrenzen. Allerdings hat IGS nicht erst mit der Einführung der minimal-invasiven Chirurgie begonnen, denn beispielsweise die Einrichtung eines Knochenbruchs unter Röntgendurchleuchtung, die schon seit Jahrzehnten praktiziert wird, fällt eigentlich auch schon unter den Begriff der bildgestützten Chirurgie. Erweitert man den Begriff zur bildgestütz-ten Intervention, dann wird die Liste der Einsatzbereiche noch länger: Ultraschallgesteuerte Punktionen oder endoluminale Endoskopien mit Röntgenkontrolle sind dafür frühe Beispiele. Im engeren Sinn spricht man aber heute dann von bildgestützter Intervention, wenn diese auf der digitalen Verarbeitung großer Datenmengen beruht. Bildgestützte Chirurgie (image guided surgery, IGS) beschreibt die Benutzung digitaler Bildinformationen für die Durchführung chirurgischer Interventionen und ist somit ein Aspekt der computergestützten Chirurgie. Durch die zunehmende Leistungsfähigkeit moderner Rechner wird das Potenzial der IGS weiter wachsen. IGS ist eine Grundvoraussetzung für die weitere Traumareduktion in der interventionellen Viszeralmedizin.Beispielhaft kann das anhand der sogenannten "Augmentierten Realität" aufgezeigt werden, wobei derzeit jedoch noch technische Probleme mit der Referenzierung zu überwinden sind. Weitere interessante Konzepte stellen die intrainterventionelle molekulare Diagnostik und die "Sentinel" Lymphknoten Detektion dar. Mittelfristig wird IGS wahrscheinlich zur Verlagerung von konventionell-offenen Eingriffen in den Bereich der minimal-invasiven Chirurgie oder sogar der interventionellen Gastroenterologie führen.Abstract !
