Back ground In this prospective randomized clinical study, we report results of management of complex tibial plateau fracture shatzker five and sex (type 41C) by Ilizarov external fixator in comparison with internal fixation according to three column classification with average follow up 18 months Patients and Methods In this study we managed 40 cases with complex tibial plateau fracture shatzker five and sex (type 41c). Twenty cases were surgically managed by Ilizarov technique and the other twenty cases were surgically managed by double internal fixation. We compare between both group as regard clinical, radiological results and rate of complications. Results while using Knee society score, the results are the following Regarding ilizarov group the range of knee society score is 31-90 with Mean±SD 76.05±14.86 (11 excellent, 4 good, 3fair, 2 poor) while group with internal fixation the range of knee society score is 44-93 with Mean±SD 73.95±13.38 (8 excellent, 5 good, 3 fair, 4 poor).According to these results; p value is 0.641 with no significant difference between two groups. Conclusion ilizarov is superior when compared with internal fixation as by ilizarov we can achieve more biological fixation especially when there highly comminuted fractures Complications rate was higher in internal fixation group (7cases) while external fixation group complications were presented in (3 cases).
Background In this prospective randomized clinical study, we report results of management of type C2 and C3 distal femur fractures by Ilizarov external fixator in comparison with double plating through separate medial and lateral approaches with a mean of 42.8 ± 6.84 weeks follow-up. Patients and Methods In this study we managed 30 cases with highly comminuted distal femoral fractures AO classification type C2 or C3. Fifteen cases were surgically managed by Ilizarov technique and the other fifteen cases were surgically managed by double plating technique. We compare between both group as regard clinical, radiological results and rate of complications. Results while using Knee society score, the results are the following: In Ilizarov group: 7 cases (46.67%) are excellent, 4 cases (26.67%) are good, 3 cases (20.00%) had fair results while 1 case (6.67%) had poor results. In internal fixation group, 8 cases (53.33%) had excellent results, 3cases (20%) had good results, 2 cases (13.33%) had fair results while 2 cases (13.33%) had poor results. Conclusion In fixation of complex distal femur fracture, both Ilizarov and double plating methods had no significant difference in clinical outcome by knee society score and in rate of complications. Ilizarov allow earlier weight bearing and less blood loss while double plating gives better ROM of knee joint and rapid radiological healing. Level of Evidence Level I Randomized controlled study.
Background:The rate of complex fracture non-unions is increased. Infected non-united fractures need more search for the best method of management.Aim of the work: to compare one-stage protocol (external fixation) and two-stage protocol (debridement, removal of metal, and internal fixation after eradication of infection) in cases of infected non-united fracture femur.
