Проанализированы результаты лечения 120 больных с диагнозом рака прямой кишки, которым выполнялась классическая тотальная мезоректумэктомия (ТМЭ) в сочетании с уни-или билатеральной тазовой лимфодиссекцией (ЛТЛД). 35 из них прошли нХЛТ, 85 пациентов -нет. С целью получения однородных сравниваемых групп больных выполнена выборка по принципу «случай-контроль». В результате образовались две группы по 32 пациента, которым выполнялась ТМЭ+ЛТЛД (контрольная группа) и нХЛТ+ТМЭ+ЛТЛД (группа сравнения). Не было получено статистически достоверной разницы в частоте поражения ЛЛ с и без нХЛТ (25 % (8/32) и 28,1 % (9/32) соответственно, р=0,08). Также не было разницы в трехлетней общей и безрецидивной выживаемости (p=0,076 и р=0,783 соответственно). Таким образом, нХЛТ не обеспечивает полную элиминацию пораженных ЛЛ, требует их удаления. нХЛТ не влияет на показатели трехлетней общей и безрецидивной выживаемости у больных раком прямой кишки. Уменьшение размера латерального лимфоузла после нХЛТ до размера менее 5 мм не свидетельствует об отсутствии метастатического поражения.

The results of treatment of 120 patients diagnosed with rectal cancer who underwent classical total mesorectumectomy (TME) in combination with uni-or bilateral pelvic lymphodissection (LPLD) were analyzed. 35 of them underwent nCRT, 85 patients did not. In order to obtain homogeneous compared groups of patients, a sample was carried out according to the «case-control» principle. As a result, two groups of 32 patients were formed, who underwent TME+LPLD (control group), and nCRT+TME+LPLD (comparison group). There was no statistically significant difference in the incidence of LL with and without nCRT [25 % (8/32) and 28.1 % (9/32), respectively, p=0.08]. There was also no difference in three-year overall and relapse-free survival (p=0.076 and p=0.783, respectively). Thus, nCRT does not provide complete elimination of affected LL, requires their removal. nCRT does not influence the indicators of three-year overall and relapse-free survival in patients with rectal cancer. A decrease in the size of the lateral lymph node after nCRT to a size of less than 5 mm does not indicate the absence of metastatic lesion.
