El estado actual de la cirugía de cabeza y cuello, producto de una constante evolución, impulsada por manos hábiles y mentes creativas e investigadoras, ha logrado marcar la historia de la otorrinolaringología y la medicina en un área con gran trascendencia y con un crecimiento acelerado en corto tiempo. Con este argumento y con la colaboración del doctor Carlos Simón Duque, se ha decidido presentar una edición especial de la revista, dirigida a aquellos médicos cirujanos y otorrinolaringólogos que se dedican a esta área específica, ya quienes con gran interés y conocimiento hacen parte de este grupo de profesionales que desarrollan e impulsan esta área del conocimiento. Por ello, en este número se encontrarán en su mayoría artículos referentes al área de la cirugía de cabeza y cuello.
La disfonía es uno de los motivos de consulta más frecuente en docentes y cantantes, quienes presentan un índice alto de patología de voz. Son muy pocos los estudios en nuestro medio acerca de la patología laríngea en la población descrita a pesar de conocerse su alta probabilidad de lesiones laríngeas. Objetivo: Describir las características estroboscópicas y factores de riesgo asociados en pacientes con abuso de voz (cantantes y docentes) que consultaron entre agosto de 2006 y noviembre de 2009 por disfonía crónica y se encontraron con lesiones benignas de laringe. Materiales y métodos: Se realizó un estudio de corte transversal, de tipo retrospectivo. Resultados: 365 pacientes fueron incluidos en el estudio, 302 docentes y 63 cantantes. Se describen la distribución de género, la frecuencia fundamental, los hallazgos glóticos la periodicidad, la simetría, onda mucosa y factores de riesgo asociados. Conclusión: La frecuencia de presentación de patología laríngea en pacientes con abuso vocal, en nuestro medio es muy alta. Encontramos cambios asociados a RGE y los nódulos vocales como principales diagnósticos en este grupo de pacientes. De igual manera es mayor la presencia de patología laríngea en personas con varios años de desarrollo de la profesión. Factores como exposición a polvo de tiza, cigarrillo, estrés, falta de educación vocal, periodos prolongados de sobreesfuerzo vocal, llevan a un aumento en la presentación de este tipo de patología.
Los avances en la tecnología nos han llevado a descubrimientos científicos importantes, a descubrir causas y tratamientos de enfermedades que antiguamente solo eran mitos y suposiciones y en las últimas décadas hemos tenido en el campo de la otorrinolaringología nuevos elementos que nos llevan a mejores desempeños en el campo laboral, no es futurista hablar de robots, de cirugía de telepresencia, y cibernética es lo que actualmente estamos viviendo, es el hoy y el mañana de la medicina. Es por ello que en el presente artículo hemos querido plasmar un campo desconocido aun por muchos y por otros realmente exaltado es el campo de la robótica, la nanotecnología y la singularidad, su aplicaciones en la otorrinolaringología, algunas de sus ventajas y desventajas. Puede que para nosotros sea el comienzo pero para nuestros hijos y futuros colegas sea la base fundamental de su vida profesional.
Las publicaciones científicas tienen como principal objetivo mantener una comunicación veraz sobre las investigaciones que producen todos y cada uno de los autores, y los lectores pretenden extrapolarla a sus respectivas poblaciones. Por ello, en respuesta a peticiones de los editores de diferentes revistas a nivel mundial, se crearon las directrices de buenas prácticas en publicaciones científicas, basadas en una gama de cuestiones éticas cada vez más complejas. Existen en la actualidad códigos de conducta para editores y autores derevistas, donde prima la libertad de expresión, la integridad, la justicia, la honestidad y la transparencia, supervisados por un ente internacional denominado COPE (Committee on Publication Ethics), encargado de mantener legislaciones éticas y proveer códigos y recomendaciones para mejorar lacalidad de las publicaciones científicas. COPE fue fundada en 1997, para hacer frente a las violaciones de la investigación y la ética de la publicación. Su objetivo es encontrar formas prácticas de hacer frente a los problemas generados en la investigación, y desarrollar buenas prácticas de la misma.
