La adecuada selección y estudio de las parejas que ingresan a los Programas de Reproducción Asistida garantiza los resultados en cuanto a tasas de embarazo y bebes en casa. La preparación de estas parejas consta en la mujer de un estudio cuidadoso de la función endocrina como es el conocer la reserva ovárica, el descartar patología utero-tubárica como el hidrosalpinx, la endometriosis, alteraciones del canal endocervical o de la cavidad endometrial. En el varón la presencia de alteraciones de los parámetros seminales pueden conducir desde una falla en la fertilización hasta infección en los sistemas de cultivos, y su cuidadosa evaluación optimiza los resultados. Siguiendo este protocolo estricto hemos logrado incrementar de manera importante nuestras tasas de gestación en un 13% (de 21% en 1995 y 1996 a 34% en 1997) por ciclo, lo que nos motiva a presentarlo.
Introducción: el objetivo de este estudio fue describir el comportamiento clínico y de laboratorio presentado en autotrasplante de células progenitoras hematopoyéticas en un grupo de pacientes del Hospital Militar Central en el periodo de enero 2007 a octubre de 2010.
Material y métodos: es un estudio descriptivo retrospectivo a través de la revisión de las historias clínicas de estos pacientes obteniendo la información sobre tiempo de alta pos trasplante, requerimiento transfusional, periodo aplásico (neutropenia absoluta, trombocitopenia severa, anemia severa y presentación de neutropenia febril), número de células madre trasplantadas, fuente y criopreservación o no. Los resultados de las diferentes variables por medir fueron tabulados y almacenados en una base para su posterior análisis descriptivo univariado cualitativo y cuantitativo.
Resultados: todos los pacientes injertaron adecuadamente. El injerto de neutrófilos ocurrió entre 11-23 días, con una media de 14.44; el injerto de plaquetas ocurrió entre 13-25 días, con una media de 16.33, y la duración de la neutropenia absoluta fue de 5-14 días, con una media de 7.28.
Conclusiones: la recuperación de los pacientes fue similar a lo descrito en la literatura internacional. (Acta Med Colomb 2012; 37: 172-176)
El Síndrome de Turner (ST) es la patología más frecuente que compromete los cromosomas sexuales, es causada por la ausencia completa o parcial del cromosoma X. Las implicaciones reproductivas de estos pacientes que se constituyen en infertilidad por una falla ovárica prematura y disgenesia gonadal, sugieren que el manejo indicado es la donación de óvulos asociada con la generación de embriones in vitro y su posterior transferencia, previa preparación endometrial. En este artículo se informan dos casos de ST manejados con ovodonación y sus implicaciones clínicas en el embarazo: Desproporción cefalo-pélvica por talla baja, complicaciones cardiovasculares que generan linfedema, lesiones aórticas y preeclampsia.
