М Ма ат те ер ри иа ал лы ы и и м ме ет то од ды ы.. Прооперированно (1987-2003 гг.) 311 пациентов с идиопатическим сколиозом (ИС). В 286 случаях использовался инструментарий Tanchev-Stefanov (TSI) и в 25-инструментарий Cotrel-Dubousset (CDI). Для сравнения включили контрольную группу из 50 пациентов, пролеченных до 1987 г. с использованием инструментария Harrington (HI). Таким образом, выборка состояла из 361 пациента. Для обеспечения достоверности отдаленных результатов ретроспективно оценивались только пациенты, оперированные до 31 декабря 1998 г. (n = 265). В исследование включены следующие системы использованных имплантатов: HI (n = 50), TSI (n = 190) и CDI (n = 25). В среднем дооперационный угол во фронтальной плоскости равнялся 72, 67 и 79° соответственно. Средние сроки наблюдения составили соответственно 180, 70 и 67 мес. При анализе оценивались материалы клинических и рентгенологических исследований, степень костного сращения, частота осложнений, субъективная оценка пациента, характеристики использованных имплантатов, эффект торакопластики и экономическая эффективность. Окончательная коррекция во фронтальной плоскости с помощью HI составила 24,5 %, что значительно ниже (р > 0,05), чем в группах с применением TSI (43 %) и CDI (45 %); эти группы показали статистически равные результаты. Сагиттальная коррекция была также сравнима-36 и 34 % соответственно (р > 0,05). Незначительный деротационный эффект (4,5°) наблюдался в группах TSI и CDI. Первичное костное сращение в группе HI получено в 84 % случаев, в группе TSI-в 95 %, в группе CDI-в 92 %. Осложнения чаще происходили при использовании HI (30 %), реже-при TSI (14 %) и CDI (12 %). Background: Lymphoma in a single bone without any nodal or visceral involvement is known as primary intra-osseous lymphoma, which is a very uncommon malignancy. It constitutes 3.1% of malignant bone tumours and 5% of extranodal lymphomas, and the incidence is only 0.6% in the jaws. These tumours can be primary or secondary (in the disseminated forms). The primary intra-osseous lymphoma of the jaws is a challenge to diagnose, because of their various clinical presentation, which can mimic an odontogenic tumour, cyst or a fibro-osseous lesion.Purpose: This case report discusses a rare malignancy of jaw bones, including clinical presentation, histopathologic features, immunologic profile, PET scan, management and prognosis.Material and methods: We present a case of a primary intraosseous lymphoma in a 45-year-old female who presented with a swelling in the right maxilla. It highlights the importance of recognizing rare entities that may present in the jaws, the impact of the disease and its management.Results: Jaw localization of Non-Hodgkin's lymphoma is rare. Clinical symptomatology and radiological signs are poorly contributive. The diagnosis relies on the histopathological analysis. Conclusion:One of the diagnostic problems with regard to NHL is the variable nature of clinical symptoms. A biopsy allowed diagnosing an intra-oral bone lymphoma, and the patient was referred to the hematooncology unit for treatment.
Background: Odontomas are mixed odontogenic tumors composed of mineralized tissue of ectomesenchymal origin. According to the World Health Organization (WHO) ranks, the odontomas are classified into two main types: complex and compound. They are usually detected in routine radiographs and may be related to various causes, and are rarely associated with impacted teeth. Purpose: The aim is to present two clinical cases of complex odontoma that prevent appropriate eruption of permanent teeth. Material and methods:We present two cases of complex odontoma associated with a retained permanent tooth. The orthopantomography and CBCT revealed circular and ovoid in shape amorphous radiopaque masses, surrounded by a thin radiolucent zone in association with an impacted permanent tooth. Results: The treatment of choice was extraction of the primary first molar, enucleation of the odontoma and the preservation of the first premolar. The complete removal of the complex odontoma was successfully performed, since after few months of follow-up the maxillary permanent teeth resumed their eruptive process. The result of the biopsy confirmed cases of complex odontoma. Conclusion: Odontomas are tumours of the dental tissues and may interfere with the eruption of the associated tooth. The early diagnosis, followed by a proper treatment at the right time, will result in a favorable prognosis and a desirable development of occlusion.
