INTRODUCTIONEmergency room is one of challenges rooms in regard to the resulting of distress and anxiety of patients and their relatives. The anxiety level in family members is varied, including depression. 1 The impact of distress and anxiety are destruction of communication among family members and these will result to inability to concentrate and insufficiency of coping mechanism.2 As a result, the changes of emotional status of the family members are ABSTRACT Background: Critical condition of patient is trigger to the increasing of anxiety of family members. This situation is vulnerable of ineffective nursing care services provide by nurses in emergency room. Communication is one of abilities which nurse have to performed, and is recognized not that simple in its implementation. This study aimed was to identify whether or not there is a relationship between communications performed by the nurses and the level of anxiety of family members of patient in Emergency Room dr. Dradjat Prawiranegara hospital, Serang Banten, Indonesia. Methods: This study was cross-sectional study with non-parametric analysis data used was Spearman's Rho. Data were collected through direct observation by 4 numerators and observational sheet of nurse's communication. Level of anxiety of family members was measuring by using State Trait Anxiety Inventory Y-1. Data were collected from 47 family members and 47 nurses who were selected by using accidental sampling technique. Results: The study revealed that nurses performed eye contact, well informed consent, quick respond to the needs of patient and family members, clear voice, understandable language, introducing them self, active listening and emphatic, calm and friendly. Regarding anxiety of family members, 42.6% of them were showing moderate level of anxiety, whereas 34.0% of them were showing low level of anxiety. In bivariate analysis by using Spearman's Rho showed that p value was 0.000 and coefficient correlation -0.765. It was indicated that there was significantly relationship between communication of emergency nurses and the level of anxiety of family members. It was indicated that the more effective the communication performed by nurses, the less the anxiety level of family members. Conclusions: Nurse is required to be able to perform their therapeutic communication skill effectively so that the anxiety level of family members is decreasing. As a result, the stable emotion of family members affect to the appropriate decision making of medical treatment.
Background: Caring for a critical ill patient are challenges for nurses. The nurse is required to deliver comprehensive nursing care including mental, psychological and physical. Caring behavior of nurse is influenced by several factors both internal and external. Objectives: This study aimed was to examine the relationship between nursing supervision, spirituality, caring efficacy, emotional quotation, work stress and working periods with caring behavior while deliver care for a critical patient. Methods: Cross-sectional design was used with the number of participants were 66 nurses. Data were collected in July 2018 in three wards: emergency room, HCU, and ICU in dr. DradjatPrawiranegara Hospital, Banten. Instruments used were the Spiritual Orientation Inventory questionnaires, Manchester Clinical Supervision Scale, Caring Behavior Assessment, and Caring Efficacy Scale which were modified and tested for validity and reliability. Data were analyzed using multivariate linear regressions. Results: The results showed that there were significant relationships between nursing supervision(r=0.511;p<0.01),spirituality(r=0.448;p<0.01),caringefficacy(r=0.433; p<0.01) and emotional intelligence (r = 0.419; p<0.01) with nurse caring behavior in care critical patients (p<0.001). There was no significant relationship between Work Periodandworkstresswithcaringbehavior(p>0.05). Final statistical analysis by using multiple regression showed that nursing supervision Beta=0.434;p<0.01),spirituality (Beta = 0.312; p<0.01) and caring efficacy (Beta = 0.297; p<0.01) were recognized as predictors of nurse’s caring behavior in care critical patient ward. Conclusion: The Increasing of nursing supervision as an external effort and developing activities are expected to improve spirituality and caring efficacy of nurse is needed to improve nurse’s caring behavior in caring for a critical patient.
AbstrakPerawatan di ICU dapat menandakan ancaman bagi pasien yang dirawat di unit tersebut, besarnya ancaman kehidupan di ICU tidak hanya menimbulkan kecemasan bagi pasien namun juga bagi keluarga. Keluarga berperan penting dalam upaya penyembuhan dan pemulihan kesehatan pasien untuk itu pihak rumah sakit juga perlu memperhatikan aspek kebutuhan keluarga pasien. Penelitian ini bertujuan untuk mengetahui gambaran pemenuhan kebutuhan keluarga pasien kritis di ICU RSUD dr. Dradjat Prawiranegara Serang Tahun 2017. Desain dalam penelitian ini adalah deskriptif dengan pendekatan kuantitatif. Penelitian ini dilakukan di ICU RSUD dr. Dradjat Prawiranegara Serang dengan jumlah sampel 40 responden. Hasil penelitian ini menunjukan bahwa (55,0%) responden mengatakan kebutuhan keluarga pasien di ICU terpenuhi dan sebesar (45,0%) responden mengatakan bahwa kebutuhan keluarga pasien di ICU belum terpenuhi. Kebutuhan informasi merupakan kebutuhan keluarga yang paling banyak terpenuhi (65,0%), sedangkan kebutuhan dukungan mental merupakan kebutuhan keluarga yang paling sedikit terpenuhi (57,5%). Kesimpulannya pemenuhan kebutuhan keluarga pasien di ICU masih perlu ditingkatkan lagi utamanya pada dukungan mental. Sehingga upaya yang bisa dilakukan untuk memenuhi kebutuhan dukungan mental adalah dengan mengaplikasikan praktik keperawatan secara holistik yang dimana diperlukan pendekatan antara petugas ICU dengan keluarga dalam konteks family center care.Kata Kunci: Intensive Care Unit, Kebutuhan Keluarga, Pasien Kritis. AbstractTreatment in the ICU can signal a threat to patients treated in this unit, the magnitude of life threats in the ICU not only raises anxiety for patients but also for families. Families play an important role in healing and restore the health of patients to the hospital also need to pay attention to aspects of family needs of patients. This study aims to find out the description of the fulfillment of family needs of critical patients in ICU RSUD dr. Dradjat Prawiranegara Serang Year 2017. Design in this research is descriptive with quantitative approach. This research was conducted in ICU RSUD dr. Dradjat Prawiranegara Serang with a total sample of 40 respondents. The results of this study showed that (55.0%) of respondents said the needs of the patient's family in the ICU is fulfilled and amounted to (45.0%) respondents said that the patient's family needs in ICU has not been fulfilled. The need for information is the most fulfilled family needs (65.0%), while the need for mental support is the most unmet family needs (57.5%). In conclusion, meeting the needs of the patient's family in ICU still needs to be improved especially on mental support. In order for the effort that can be made to meet the needs of mental support is to apply a holistic nursing practice where an approach is needed between the ICU staff and the family in the context of family care center.
