Aims. Optimal ECG monitoring in detecting recurrences of atrial fibrillation (AF) or atrial tachycardia (AT) after catheter ablation has not been well established. The purpose of this prospective study was to compare the utility of daily ECG monitoring with episodic card recorder (ECR) vs. periodic monitoring with episodic loop recorder (ELR) for the detection of post-blanking AF/AT recurrences during early (Months 4-6) and late (Months 7-12) periods after catheter ablation for paroxysmal AF. Methods. The study included 105 consecutive patients, who received ECR for 12 months and were instructed to send at least 2 random ECG recordings daily with extra-recordings during symptoms. The patients were simultaneously monitored for one week with ELR at the end of each period (Months 6 and 12). Results. Thirty-one and 12 patients with AF/AT recurrence were identified by means of ECR and ELR, respectively. In patients with complete and valid data, ELR technology was inferior to ECR by detecting AF/AT in 5 (31%) of 16 and 5 (26%) of 19 patients with arrhythmia identified by ECR in the early and late period, respectively. Overall, ELR had a sensitivity of 8/23 (35%) for detecting AF/AT recurrence. There was no single patient with AF/AT recurrence on ELR that would not be known from ECR monitoring. Only 2 patients with arrhythmia recurrence were completely asymptomatic throughout the study period. Conclusion. Daily ECG monitoring with ECR was better than periodic monitoring with ELR in detecting AF/AT recurrences during the follow-up periods. Entirely asymptomatic patients with AF/AT recurrences were rare.
Úvod: Srdeční resynchronizační léčba (SRL) hraje významnou úlohu v terapii srdečního selhání u pacientů s nízkou ejekční frakcí. Nicméně, signifi kantní část pacientů se po implantaci SRL přístroje nezlepší. Optimalizace atrioventrikulárního a interventrikulárního intervalu může pomoct zlepšit klinický stav pacientů. Cíl: Cílem práce je porovnat optimalizaci atrioventrikulárního a interventrikulárního intervalu pomocí neinvazivního měření plochy pod křivkou transaortálního průtoku (aortic VTI) a pomocí invazivního měření maximální hodnoty první derivace tlaku v levé komoře (LVdP/dt max ) a pomocí algoritmu QuickOpt. Metodika: Patnáct non-responderů bylo optimalizováno neinvazivně -pomocí aortic VTI v porovnání s metodou QuickOpt. Kontrolní vyšetření bylo provedeno za tři měsíce: klinické a echokardiografi cké vyšetření. Deset non-responderů bylo optimalizováno invazivně -pomocí LVdP/ dt max v porovnání s metodou QuickOpt. Kontrolní vyšetření bylo provedeno za tři měsíce: klinické a echokardiografi cké vyšetření. Výsledky: Ve skupině s neinvazivní optimalizací (věk 74,3 roku [65,3; 84,3], 83,4 % mužů, etiologie: 66 % ischemická choroba srdeční, třída NYHA před optimalizací III v 66,7 %, III-IV ve 33,3 %, ejekční frakce levé komory 23,0 % [15,0; 32,0]) jsme nepotvrdili korelaci optimálního atrioventrikulárního a interventrikulárního intervalu zjištěného pomocí obou metod. Optimalizace měla vliv na zkrácení komplexu QRS. Po třech měsících sledování nebyla nalezena statisticky signifi kantní změna v ejekční frakci a jiných parametrech ultrazvukového vyšetření či třídy NYHA. Ve skupině pacientů s invazivní optimalizací (věk 76,7 roku [66,6; 82,4], 90 % mužů, etiologie: 60 % ischemická choroba srdeční, třída NYHA před optimalizací III v 50 %, III-IV v 50 %, ejekční frakce levé komory 29,5 % [10,0; 35,0]) jsme nepotvrdili korelaci optimálního atrioventrikulárního a interventrikulárního intervalu zjištěného pomocí obou metod. Optimalizace měla vliv na zkrácení komplexu QRS. Po třech měsících jsme zjistili zlepšení ejekční frakce levé komory, ale ostatní echokardiografi cké parametry či třída NYHA zůstaly nezměněny. Závěr: Použití měření plochy pod křivkou transaortálního průtoku při optimalizaci srdeční resynchronizační léčby nepřineslo pacientům non-responderům užitek -echokardiografi cké parametry ani třída NYHA se při tříměsíčním sledování nezměnily. Použití invazivního měření maximální hodnoty první derivace tlaku v levé komoře vedlo ke zlepšení ejekční frakce, třída NYHA však zůstala nezměněna.

Background Pulsed field ablation (PFA) is a promising method to reduce procedure time and improve ablation safety and efficacy in all types of atrial fibrillation (AF). Purpose To present ablation approach and medium-term outcomes of PFA in all AF types. Methods Between 9/2021-10/2022, PFA using Farawave catheter was performed in 250 patients (63±11 years, 65 females). Patients with paroxysmal AF were basically scheduled for simple pulmonary vein isolation (PVI) (Farawave, ICE), patients with longstanding AF (LSPAF) underwent complex left atrial (LA) PFA (Farawave, CS 10-pole catheter, ICE); ablation extent varied in persistent AF. Results Paroxysmal AF (n=131; 115 ablated in SR), persistent AF (n=39; 11 ablated in SR), and LSPAF (n=64) were ablated with procedure times 58±20, 105±28, and 116±32 minutes, respectively, and fluoroscopy times 9±5, 14±7, and 17±7 minutes, respectively. Additional 16 patients underwent PFA for atrial tachycardia (AT). Ongoing AF was terminated by PFA in 13/16 (81%), 16/28 (57%), and 23/64 (36%) patients with paroxysmal AF, persistent AF, and LSPAF, respectively. In groups of paroxysmal AF ablated in SR, paroxysmal AF ablated in AF, persistent AF ablated in SR, persistent AF, and LSPAF, simple PVI was performed in 100%, 50%, 73%, 11%, and 0 patients respectively; and freedom from AF/AT was present in 86/99 (87%), 11/14 (79%), 7/10 (70%), 11/12 (92%), and 40/52 (77%) patients, respectively, of total 187 patients with follow-up ≥3 months (≥6 months in 139 patients). In 8 patients with repeat ablation post-PFA for paroxysmal AF (n=4), persistent AF (n=2), LSPAF (n=2), extra-PV AF/AT sources dominated in all groups. Complications included one tamponade with pericardiocentesis, and prolonged ST elevation with negative coronary angiography. Conclusion PFA using Farawave catheter was associated with reduced procedure times and favourable medium-term outcome for all AF types. Early experience specifically suggests improved outcome after extensive LA ablation for persistent AF/LSPAF.
