AimsTo evaluate the efficacy and toxicity of extracranial stereotactic body radiotherapy (SBRT) in the treatment of oligometastatic lymph node involvement in the mediastinum, retroperitoneum, or pelvis, in a consecutive group of patients from real clinical practice outside clinical trials.MethodsA retrospective analysis of 90 patients with a maximum of four oligometastases and various primary tumors (the most common being colorectal cancers). The endpoints were local control of treated metastases (LC), freedom from widespread dissemination (FFWD), progression-free survival (PFS), overall survival (OS), and freedom from systemic treatment (FFST). Acute and delayed toxicities were also evaluated.ResultsThe median follow-up after SBRT was 34.9 months. The LC rate at three and five years was 68.4 and 56.3%, respectively. The observed median FFWD was 14.6 months, with a five-year FFWD rate of 33.7%. The median PFS was 9.4 months; the three-year PFS rate was 19.8%. The median FFST was 14.0 months; the five-year FFST rate was 23.5%. The OS rate at three and five years was 61.8 and 39.3%, respectively. Median OS was 53.1 months. The initial dissemination significantly shortened the time to relapse, death, or activation of systemic treatment—LC (HR 4.8, p < 0.001), FFWD (HR 2.8, p = 0.001), PFS (HR 2.1, p = 0.011), FFST (HR 2.4, p = 0.005), OS (HR 2.2, p = 0.034). Patients classified as having radioresistant tumors noticed significantly higher risk in terms of LC (HR 13.8, p = 0.010), FFWD (HR 3.1, p = 0.006), PFS (HR 3.5, p < 0.001), FFST (HR 3.2, p = 0.003). The multivariable analysis detected statistically significantly worse survival outcomes for initially disseminated patients as well as separately in groups divided according to radiosensitivity. No grade III or IV toxicity was reported.ConclusionOur study shows that targeted SBRT is a very effective and low toxic treatment for oligometastatic lymph node involvement. It can delay the indication of cytotoxic chemotherapy and thus improve and maintain patient quality of life. The aim of further studies should focus on identifying patients who benefit most from SBRT, as well as the correct timing and dosage of SBRT in treatment strategy.
Při ozařování nemocných s implantovanými srdečními elektronickými přístroji (cardiovascular implantable electronical device-CIED) hrozí reálné riziko jejich poškození běžným provozem radioterapeutických přístrojů. O poruchách se v publikacích referuje od zavedení prvních lineárních urychlovačů v 60. letech 20. století. Principy zajištění ochrany se díky hlubším znalostem a analýzám příčin a frekvence poruch postupně vyvíjejí. Nejčastěji citovaná doporučení v Evropě jsou holandská doporučení dle Hurkmanse et al, resp. z nich vycházející německá doporučení DEGRO/ DGK. V roce 2019 byla zveřejněna závěrečná zpráva Americké asociace fyziků v medicíně (American Association of Physicists in Medicine-AAPM), která je v několika zásadních přístupech odlišná, zejména co se týče akceptovatelné maximální dávky, ale i v opatřeních nutných během ozařování těchto pacientů, jako je nutnost EKG sledování pacienta v průběhu léčby. Ve světle současných poznatků AAPM taky upouští od indikací k chirurgické relokaci přístroje v případech, kdy nemohou být obecná doporučení dodržena. Cíl: Každé radioterapeutické pracoviště, které ozařuje pacienty s CIED, musí mít vypracovanou vnitřní směrnici, podle které v těchto případech postupuje. Naše práce podává ucelený přehled příčin a mechanizmů poruch implantovaných přístrojů i obou uvedených přístupů k managementu ozařování pacientů s CIED a hledá optimální řešení pro tvorbu doporučení platných v ČR.
SouhrnVýchodiska: Cílem této retrospektivní studie je zhodnotit na neselektované skupině pacientů z reálné klinické praxe efektivitu a toxicitu extrakraniální stereotaktické radioterapie v léčbě oligometastatického postižení lymfatických uzlin lokalizovaných v mediastinu, retroperitoneu a pánvi. Materiál a metody: Z celkového počtu 50 pacientů léčených v letech 2011-2017 bylo 29 mužů a 21 žen, průměrný věk byl 62 let (medián 66 let, rozmezí 25-81 let). Pacienti byli ozařováni nejčastěji v pěti frakcích; dávka byla volena podle dávkově-objemových histogramů rizikových orgánů v okolí plánovacího cílového objemu. Primárním cílem bylo zhodnocení lokální kontroly (local control -LC), doby bez progrese onemocnění (progression-free survival -PFS), doby do vícečetné diseminace neumožňující využití lokálních metod léčby (freedom from widepread dissemination -FFWD) a celkového přežití (overall survival -OS). Rovněž byla zhodnocena akutní a pozdní toxicita léčby. Výsledky: Medián bio logického ekvivalentu dávky při a/ b = 10 (BED10) byl 54 Gy (rozmezí 48-80 Gy). Medián doby sledování byl 40,4 měsíce. LC v 1 roce od ozáření byla 90 %, 3letá 75 %. Mediánu doby do lokální progrese nebylo zatím dosaženo. Pacienti ozáření vyšší dávkou měli signifikantně lepší LC onemocnění než pacienti ozáření dávkou nižší; hranicí byl medián aplikovaných dávek (tj. BED10 = 54 Gy). Lokalizace patologických uzlin na LC signifikantní vliv neměla. Medián PFS byl 8,2 měsíce (95% CI 7,4-11,6 měsíce). Jednoleté PFS bylo 38,5 % a 3leté 17 %. Medián OS byl 37,3 měsíce (95% CI 23,2-51,4 měsíce). Jednoleté OS bylo 83 % a 3leté OS bylo 51 %. Medián FFWD byl 13,6 měsíce (s rozmezím 8,7-18,5 měsíce). Jednoletá FFWD byla 55 % a 3letá FFWD 24 %. Žádný z těchto parametrů (PFS, OS, FFWD) nebyl závislý na aplikované dávce ani na lokalizaci metastáz. Nebyla zaznamenána žádná toxicita III. a IV. stupně. Závěr: Naše studie prokázala, že cílená stereotaktická radioterapie je v případě oligometastatického postižení lymfatických uzlin minimálně toxickou a vysoce efektivní lokální metodou léčby. Je schopna s minimálním úsilím pacienta oddálit indikaci cytotoxické chemoterapie a tím event. zlepšit kvalitu života pacientů. Necelá pětina takto léčených pacientů přežívá bez známek onemocnění dlouhodobě. Identifikace pacientů, kteří mají z této léčby největší prospěch, správné načasování extrakraniální stereotaktické radioterapie v léčebné strategii, popř. velikost dávky by se měly stát předmětem dalších studií.
