INTRODUÇÃO: A colecistectomia, remoção da vesícula biliar, é um procedimento cirúrgico comum realizado como forma de tratamento para doenças biliares em pacientes sintomáticos. De forma geral, a cirurgia deve promover a melhora da cólica biliar; entretanto, alguns dos sintomas já préexistentes e/ou outros novos sintomas podem surgir após a intervenção, sendo caracterizados como Síndrome Pós-Colecistectomia (SPC). Nesse contexto, a principal manifestação da SPC é a Diarreia Pós-Colecistectomia (DPC), a qual é definida como a presença de fezes líquidas ou amolecidas e/ou como o aumento na frequência de evacuações, após a intervenção cirúrgica. Por ser uma patologia bastante frequente, presente em 12 à 57,2% dos colecistectomizados, propôs-se realizar essa revisão bibliográfica a fim de ampliar o conhecimento acerca da DPC. OBJETIVO: Este estudo tem como objetivo ressaltar os aspectos fisiopatológicos e o tratamento, medicamentoso e não-medicamentoso, para a DPC, facilitando a conclusão diagnóstica da mesma e antecipando o tratamento da doença, a fim de reduzir a ocorrência de efeitos adversos. MÉTODOS: Trata-se de uma revisão de literatura, realizada a partir de 9 referências, nas línguas Espanhol, Inglês e Português, do período de 2016 à 2021, acerca da fisiopatologia e do tratamento para a DPC. As bases de dados escolhidas foram o LILACS e o MEDLINE. RESULTADOS E DISCUSSÃO: Estudos apontam o mecanismo patogênico da DPC a partir da síntese exacerbada de ácidos biliares, devido à interrupção do equilíbrio dos mesmos, associada à excessiva desidroxilação bacteriana no intestino delgado, a qual dificulta a absorção dos ABs. Assim, a síntese e a absorção desreguladas fazem com que os ácidos alcancem constantemente o intestino grosso e fiquem por mais tempo em contato com a mucosa, acarretando em alterações na microbiota intestinal e em uma resposta inflamatória crônica, o que resulta em mudanças do hábito intestinal e na diarreia por sais biliares. Ademais, pesquisas têm demonstrado que uma dieta hipo-lipídica associada à administração de sequestradores de ácidos biliares possuem grande eficácia no tratamento da DPC. A escolha do representante dessa classe à ser utilizado deve ser realizada de forma individualizada, uma vez que é levado em consideração a eficácia, a tolerância do paciente aos efeitos adversos do medicamento e, também, o preço de custo desses. Além disso, incorporar o uso de probióticos com o objetivo de regular a microbiota intestinal, parece uma opção válida e bastante conveniente no contexto dessa patologia. CONCLUSÃO: Conclui-se que reconhecer os principais mecanismos patogênicos da Diarreia Pós-Colecistectomia permite um melhor manejo do paciente e, assim, a realização da adequada conduta terapêutica, a qual visa reduzir os efeitos adversos da doença e melhorar a qualidade de vida do paciente. Introdução: A Nefropatia por IgA, doença renal imunomediada indireta, é considerada a principal etiologia das glomerulopatias e, também, é responsável por grande parte de todos os casos de doença renal crônica em estágio terminal. Objetivo: Relatar o caso de uma paciente internada em ambiente hospitalar com apresentação clínica de síndrome mista e presença de vasculite em membros inferiores, com hipótese diagnóstica de Nefropatia por IgA. Metodologia: As informações foram obtidas através da revisão do prontuário, registro fotográfico dos métodos diagnósticos aos quais a paciente foi submetida e revisão da literatura. Discussão: Estudos apontam o mecanismo patogênico a partir da deposição de imunocomplexos circulantes no glomérulo renal, levando à inflamação e à ativação do sistema imunológico. A apresentação clínica é variável, sendo a síndrome mista a mais frequente. O diagnóstico é realizado através da biópsia renal, sendo a histologia avaliada conforme a Classificação de Oxford da Nefropatia de IgA. Como tratamento orienta-se o controle dos fatores de risco, administração de anti-hipertensivos nefro-protetores, estatina e, em casos específicos, corticoterapia. Conclusão: A conclusão diagnóstica é realizada somente através da biópsia renal, por meio da imunofluorescência; entretanto, a história e os achados clínicos costumam ser bastante sugestivos e suficientes para iniciar o tratamento empírico.
