RESUMOSomente a partir de 1960, a depressão foi relacionada à infância e adolescência. A importância desse problema vem sendo reconhecida devido ao aumento constante de casos clínicos nesta faixa etária, frequentemente identificados pelos profissionais da saúde mental. Trata-se de um estudo de campo descritivo, intervencionista, transversal, de natureza quanti-qualitativa e com abordagem na depressão em adolescentes da Escola Estadual Guiomar de Melo. Realizado com 221 participantes de 15 a 20 anos, estudantes do 1º, 2º e 3º ano do Ensino Médio da Escola Guiomar de Melo. Sob essa ótica, observou-se que, dentre a amostra total, 87 alunos (39%) cursam o 1º ano do Ensino Médio, 52 (23,5%), o 2º ano e 82 (37,1%), o 3ºano. Após esses dados, os questionários foram classificados em graus de depressão segundo o inventário de Beck, obedecendo a seguinte pontuação: 0 a 13 pontos-nenhuma depressão(N), 14 a 19 pontos -depressão leve(DL), 20 a 28 pontos-depressão moderada(DM), 29 a 63 pontosdepressão grave(DG). Quando comparado os sexos, foi notável que o feminino possuiu mais diagnósticos de depressão, incluindo graus leve, moderado e grave, sendo 67(29%) de um total de 221 alunos, contra 29(13%) no sexo masculino, o que é compatível com a literatura. Os achados sugerem fomentar práticas educativas relacionadas à depressão, como a realização de palestras no âmbito escolar, além do direcionamento dos alunos à terapia psicológica visando à melhoria da saúde mental dos alunos. As cardiopatias, principais causas de morbimortalidade materna, incidem em 1% das gestações. Uma das raras causas, a Síndrome de Takotsubo (ST), é caracterizada por disfunção ventricular esquerda reversível. Permanece desconhecida e pobre em estudos clínicos, sendo então subdiagnosticada. Assemelha-se a síndrome coronariana aguda e, eventualmente, se apresenta como emergência cardiológica. O objetivo deste relato foi descrever o caso de uma puérpera de gestação gemelar que apresentou diagnóstico de ST. A teoria mais aceita da etiopatologia é de um aumento na liberação local de catecolaminas induzida pelo estresse, produzindo espasmos cardíacos que geram alterações na contratilidade cardíaca. Na ST, o foco é no episódio agudo, pois a disfunção é transitória. O tratamento de suporte é o mesmo que nas demais situações cardiológicas. São necessários mais estudos sobre a ST em gestantes e puérperas, de maneira a direcionar a condução de casos futuros. A abordagem pré-concepcional e perinatal das cardiopatias é importante para diminuir intercorrências que possam aumentar o risco materno e fetal.
INTRODUÇÃO: Os distúrbios hipertensivos tem alta incidência e morbimortalidade na gestação. A pré-eclâmpsia é indicada pela presença de hipertensão e proteinúria após 20 semanas. Define -se diagnóstico uma proteinúria de 24h ≥ 300mg, > que 1+ na fita, ou, relação proteinúria/creatinúria (P/C) ≥ a 0,3 em amostra isolada. Diante disso, como o padrão-ouro dura 24 horas, vários testes tentam facilitar o diagnóstico de forma rápida e confiável.OBJETIVOS: Elaborar uma revisão da literatura sobre os testes urinários usados na pré-eclâmpsia e definir a acurácia da P/C no diagnóstico da doença.METODOLOGIA: Revisão de artigos, ensaios clínicos e revisões, nas bases de dados do PubMed, Medline, BVMS/BIREME e Scielo. Com os termos “Pré-eclâmpsia”, “Proteinúria”, “Testes urinários”, foram escolhidos “10” artigos, dos anos 1997 a 2021. Usou-se como escolha: pesquisas até o ano de 2021, em português e inglês, optando por aquelas que abordaram dados epidemiológicos e formas de quantificação da proteinúria.RESULTADOS: De acordo com a fisiopatologia da doença, a disfunção placentária causa a liberação de citocinas inflamatórias e estresse oxidativo. A nível renal, ocorrem alterações na permeabilidade das células endoteliais glomerulares, impedindo a reabsorção das proteínas nos túbulos proximais. Dentre os exames, o padrão ouro continua sendo o de 24 horas. Porém, a P/C mostra boa correlação em pacientes sem comorbidades e pode ser usada por ser mais rápida e barata. Estudos comparativos mostraram que a P/C = 0,3 tem sensibilidade em torno de 87% para o diagnóstico de proteinúria de 24h ≥ 300 mg. Valores da P/C < a 0,15 afastaram a proteinúria ≥ 300mg/24h em 99% das vezes e níveis entre 0,3 e 0,15 devem ser confirmados pelo exame padrão-ouro. Foi notório, em algumas metanálises, que a P/C, em gestantes com comorbidades de acometimento renal, como a nefropatia diabética, não obteve uma acurácia linear no diagnóstico, com mais falso positivos. Além disso houve oscilação de perda de proteínas durante o dia e volume de urina insuficiente para a coleta, que podem ter interferido no resultado. Porém, sabe-se que muitas vezes o obstetra necessita de decisões imediatas, que não podem esperar a coleta de 24horas.CONCLUSÃO: Com essa análise, é essencial dizer que a P/C ainda é menos confiável que a proteinúria de 24 horas. Mas seu uso é sugerido em locais com menos recursos financeiros ou em casos de urgência. Se possível, é fundamental a confirmação, com o exame padrão-ouro.
