O ensino de habilidades de comunicação clínica na graduação médica encontra nos princípios e componentes do Método Clínico Centrado na Pessoa (MCCP) uma referência importante para a definição de suas competências. No entanto, mesmo tendo contato com o MCCP em sua formação, é frequente que estudantes de medicina recorram à utilização da anamnese tradicional centrada na agenda médica como um roteiro mais seguro para realização de suas entrevistas. Propomos, como uma hipótese para essa dificuldade dos estudantes, a falta de uma tradução do MCCP em um roteiro padronizado de entrevista médica, especialmente para ensino na graduação. Neste relato, a partir de modelos de entrevista clínica centrada na pessoa (ECCP) selecionados da literatura internacional, apresentamos a primeira etapa de um roteiro de ECCP original, adaptado ao cenário brasileiro. O objetivo deste relato é oferecer uma referência de fácil utilização em língua portuguesa e que possa ser aprimorada pelos profissionais envolvidos com o ensino de comunicação clínica na educação superior no Brasil. Estudos empíricos ainda são necessários para endossar uma utilização mais ampla da proposta aqui apresentada.
Research carried out with the objective of knowing the project of happiness of a population deprived of liberty and its implications for health care. For this, the qualitative study was used. Data collection took place from July 2019 to October 2020, in a prison in the interior of Zona da Mata Mineira, with 20 people deprived of liberty. An open interview was carried out with a semi-structured script, being submitted to Content Analysis. Ethical aspects were respected. The results indicate that the happiness projects of people deprived of liberty find meanings, both in specific experiences of the present through the redefinition of ways of living in prison, and in future perspectives through the construction of thoughts about life after prison. They also want to carry out social projects in schools to raise awareness of young people about living in prison, as well as accessing fundamental rights such as education and work, building and caring for the family and following a religion. Thus, the subjective production mobilized in prison, produces as a project of happiness, participating in the breaking of the perverse cycle that historically imprisons those whose rights are not accessed in life outside prison, enabling the construction of care consistent with comprehensiveness and humanization.
O entendimento do processo saúde-doença muda com o tempo histórico, tendo seguido uma tendência a cada vez mais patologizar fenômenos antes tidos como naturais, abrindo caminho para um cenário de valorização de excessos. Diante disso e da importância de proteger as pessoas dos possíveis danos causados pelas intervenções médicas, surgiu a prevenção quaternária (P4), que versa sobre uma medicina menos iatrogênica, evitando sobrediagnóstico, sobremedicalização e intervenções desnecessárias. Contudo, na prática clínica, médicos encontram diversos entraves para a sua aplicação, sendo comum não utilizá-la, mesmo se tendo consciência de sua importância. Dentre essas barreiras, destacam-se a dificuldade de negar uma demanda ao paciente e de lidar com situações de discordância, a pouca destreza argumentativa, e o medo de deixar de fazer um diagnóstico ou de perder a confiança da relação. O presente ensaio discute técnicas de habilidades de comunicação, à luz da medicina centrada na pessoa, com o objetivo de auxiliar médicos e médicas diante dessas dificuldades. Partindo de situações cotidianas, foram buscadas respostas em referências científicas, como livros, artigos e manuais. Algumas dessas técnicas apresentadas são: como negar uma demanda, entender o desejo pessoal por uma intervenção médica, lidar com a divergência de ideias, explorar a experiência da não-concordância, desprescrever medicações e compartilhar as decisões. Espera-se que, a partir desta leitura, os profissionais se sintam mais preparados e motivados para nadar contra a corrente hegemônica das intervenções médicas desnecessárias, contribuindo para que a medicina avance no sentido de promover o melhor cuidado possível às pessoas, sobretudo, primeiro não lhes causando mal.
