RESUMO -Relatamos o caso de um menino com cariótipo XXY que apresenta desordem neurológica progressiva com início por volta dos 11 meses de idade, com estagnação do desenvolvimento seguida de regressão. A criança apresenta, ainda, movimentos estereotipados de mãos, apraxia manual e microcefalia. Investigações não constataram presença de qualquer condição neurológica ou sistêmica definida que pudesse ser apontada como possível etiologia para o quadro descrito. Trata-se de menino com alterações fenotípicas muito similares àquelas consideradas típicas para a síndrome de Rett que, associadas com a alteração cromossômica constatada (cariótipo XXY), constituem quadro de evidente interesse científico.PALAVRAS-CHAVE: síndrome de Rett, cariótipo XXY, síndrome de Klinefelter.
Rett syndrome phenotype in XXY karyotype: case reportABSTRACT -We report the case of a XXY boy who presents progressive neurological disorder which has started around eleven months of age, with developmental stagnation followed by regression. The child presents as well stereotypic hand movements, loss of purposeful hands use and microcephalia. Presence of any defined systemic or neurological condition which could be pointed out as the possible etiological factor for the case was not found out by investigations. It deals with a boy with phenotypic alterations very similar to those considered typical for Rett syndrome which associated with chromosomal alteration (XXY kariotype) constitute evident scientific interest.KEY WORDS: Rett syndrome, XXY karyotype, Klinefelter syndrome.A síndrome de Rett é uma condição neurológica de etiologia desconhecida, descrita por Andreas Rett em 1966, que, em sua forma clássica, afeta crianças do sexo feminino. Caracteriza-se por regressão no desenvolvimento com início por volta dos 8 a 10 meses de idade e pela presença de sintomas do tipo autístico, perda do uso práxico das mãos, movimentos estereotipados, microcefalia adquirida e evidente retardo cognitivo e de comunicação 1-3 . Pelos critérios do CID-10, é classificada como uma das assim chamadas desordens abrangentes do desenvolvimento.Os casos identificados até hoje referem-se apenas ao sexo feminino. Foram relatados alguns poucos casos em que o fenótipo similar foi observado no sexo masculino 4-8 , nos quais, entretanto, o quadro clínico descrito era, em geral, apenas sugestivo do diagnóstico da síndrome de Rett, e os sinais e sintomas estavam presentes de forma atípica e/ou parcial.
