RESUMO -No presente estudo, foram pesquisados os tumores do sistema nervoso central (SNC) diagnosticados na cidade de Curitiba entre 1990 e 1994. Realizaram-se 2371 biópsias de SNC das quais 1340 casos eram neoplasias primárias. Dentre as neoplasias primárias de SNC 304 (22,6%) eram meningiomas. Duzentos e nove meningiomas ocorreram em mulheres (68,7%) e 95 ocorreram em homens (31,3%). As idades variaram de 3 a 90 anos, com média de 45,8 anos. Duzentos e oitenta meningiomas localizavam-se no cérebro, 10 na medula espinhal, 9 no cerebelo e 5 na emergência de nervos cranianos. Na classificação histológica verificou-se que 294 (96,7%) eram do tipo clássico, 6 malignos (ou anaplásicos), 3 atípicos e 1 papilar. No grupo dos meningiomas clássicos 267 eram meningoteliais, 10 psamomatosos, 5 fibroblásticos, 5 microcísticos, 5 transicionais e 2 angiomatoses. Os autores concluem que as neoplasias meníngeas representaram o grupo mais frequente de tumores primários do SNC e apresentaram características epidemiológicas e anátomo-patológicas similares às da literatura mundial, na qual é dada ênfase à dificuldade de estabelecer critérios biológicos e histológicos de malignidade neste grupo de tumores. Meningiomas: epidemiological and pathological studies of 304 casesABSTRACT -The authors have retrospectively reviewed all tumors of central nervous system (CNS) operated at the most important neurosurgery hospitals of Curitiba in a 5-year period (1990)(1991)(1992)(1993)(1994) and found 304 (22.4%) cases of meningioma. Age mean of the patients was 48.5 years, with a range of 3 to 90 years. A marked female preponderance (68.7%) was noted. The most common tumor location was brain (n=280) and the remaining cases occurred in spinal cord (n=10), cerebellum (n=9) and cranial nerves (n=5). Histologically, there were 294 (96.7%) meningiomas of the classical type, six malignant or anaplastic, three atypical and one papillary. Two hundred and sixty seven classical meningiomas were from the meningotelial subtype, ten psamomatousos, five fibroblastic, five microcystic, five transicional and two angiomatous. O primeiro paciente tinha 34 anos por ocasião do óbito e os achados de necrópsia mostraram hemangioblastoma de retina, cerebelo, bulbo e medula espinhal, além de carcinoma renal, feocromocitoma, lesões císticas de rim e pâncreas, hidromielia e meningiomas atípicos. Seu irmão morreu com 30 anos de idade e a autópsia revelou hemangioblastomas de cerebelo, carcinoma renal e cistoadenoma de células claras de epididimo. It was characterized as a clinical entity by Lindau (1926) who described the occurrence of central nervous system and visceral abnormalities including haemangioblastomas of the cerebellum, retina and spinal cord, pancreatic and renal cell carcinoma and cysts, and phaeochromocytoma 5 .In the present study we present the post-mortem findings of two familial cases and a further case of the same family operated by cerebellar haemangioblastoma; so far no similar reports have been documented in the Brazilian literature.
RESUMO -A cirurgia para tratamento da doença discal lombo-sacra pode cursar com complicações severas, iatrogênicas incluindo-se as neurológicas, vasculares, entéricas, ureterais e disfunção sexual podendo levar o paciente à morte dependendo da extensão da lesão. Abramovitz relata que as complicações vasculares do acesso posterior para o tratamento hérnia discal lombo-sacra ocorrem em menos de 0,1% dos casos As lesões vasculares podem manifestar-se de maneira imediata como hemorragia arterial levando a choque hipovolêmico no per ou pós-operatório imediato, devendo ser diagnosticadas e tratadas prontamente. Sinais precoces de hemorragia retroperitoneal se manifestam na vigência de lesões traumáticas, sendo os pseudoaneurismas e as fístulas arteriovenosas diagnosticados tardiamente, semanas ou mesmo após anos. Descrevemos o caso de uma paciente de 39 anos que foi submetida a tratamento cirúrgico de hérnia discal no espaço L4-L5, à direita, em outro serviço. Devido ao fato de persistirem os sintomas dolorosos, foi encaminhada ao nosso serviço. Na investigação a ressonância magnética mostrou lesão paravertebral sugestiva de abscesso. A angiografia aorto-ilíaca revelou a presença de pseudoaneurisma na artéria ilíaca comum esquerda, o qual foi reparado cirurgicamente com sucesso. Commom iliac artery false aneurysm as complication of lumbar disc surgery: case reportABSTRACT -Surgery for discogenic disease of lumbar region is a challenging and diverse field. The significance of complications in disc surgery arises from the possibility that failure of primary treatment may lead to a permanently disable patient. The most common iatrogenic lesions in the posterior approach to lumbar disc herniation are neurologic, vascular, enteric, ureteral, and sexual dysfunction. Abramovitz reports that vascular complications of the posterior approach occur in at least 0.1% of cases. The vascular complications may present during surgery as severe bleeding leading to hypotension. The most common late complication is arteriovenous fistula followed by false aneurysms. We describe the case of a 39 year-old woman who was operated on for a lumbar disc herniation in other hospital. She came to us with a persistent pain in lumbar area. A MRI scan of lumbosacral region revealed a paravertebral lesion suggesting abscess. An angiography was performed and showed a false aneurysm of left common iliac artery, which was surgically treated successfully.
