Introdução: A presença de sintomas otológicos associados à desordem temporomandibular (DTM) é discutida há seis décadas; entretanto, sua etiologia ainda permanece obscura. Forma de estudo: Prospectivo clínico randomizado. Objetivo: Neste estudo foram avaliadas a prevalência de sintomas otológicos na DTM, sua correlação com a dor muscular e a ausência de dentes posteriores. Material e Método: Foram avaliados 126 pacientes portadores de DTM, através de questionário subjetivo dos sintomas, palpação dos músculos de mastigação, temporal, masséter, pterigóideo lateral, pterigóideo medial, digástrico, tendão do músculo temporal e dos músculos esternocleidomastóideo e trapézio. Foram feitas radiografias panorâmica e transcraniana e modelos de gesso das arcadas superior e inferior dos pacientes. Os dados obtidos foram analisados através do Teste Exato de Fisher, com percentil de significância menor que 0,05. Resultados: Houve presença de sintomas otológicos em 80% dos pacientes, sendo que 50% apresentavam dor referida em ouvido; 52%, plenitude auricular; 50%, tinitus; 34%, tontura; 9%, sensação de vertigem; e 10% relataram hipoacusia. O músculo pterigóideo lateral foi o músculo mais sensível em 94% dos pacientes, seguido do músculo temporal em 69%, masséter em 62%, digástrico em 60%, pterigóideo medial em 50%, tendão do músculo temporal e esternocleidomastóideo em 49% e trapézio em 42% dos pacientes. Houve significância para dor muscular, e a presença de sintomas otológicos, nos músculos masséter e esternocleidomastóideo. Os sintomas tinitus, plenitude auricular e dor referida em ouvido apresentaram alta correlação de significância entre si. Não houve significância para a ausência de dentição e sintomas otológicos. Conclusão: 1) Dor referida em ouvido, tínitus, plenitude auricular e tontura foram prevalentes; 2) os sintomas otológicos presentes na DTM podem estar relacionados com a dor muscular em masséter e esternocleidomastóideo; 3) não houve correlação entre os sintomas otológicos e a ausência de dentes posteriores.

-The prevalence of excessive daytime sleepiness (EDS) in general population was determined by means of 408 home interviews of adults, in a representative sample of Campo Grande city, Brazil. The random sample was stratified by sex, age and economic social status. EDS was considered in those with indexes 11 or more in the Epworth Sleepiness Scale. Statistics used chi-square, Fisher and Pearson tests; and inferences based on binomial distribution parameters; the significance level was 5% and confidence interval (CI) was 95%. The prevalence of EDS was 18.9% of the general population ( SD=1.9%; CI 15.1% to 22.7%). No significant association was found between EDS and the use of hypnotics, nor with insomnia, body mass index, sex, age, years of schooling, economic social status, marital status, occupation and the use of alternative means to improve sleep. A structured questionnaire was used with the consent from the interviewed subjects. Statistics used chi-square, and Fisher tests; and inferences based on binomial distribution parameters; the significance level was 5% and confidence interval (CI) was 95%.The general prevalence of insomnia was 19.1% (sd=2.0%), mostly women (p=0.0015), and people of less years of schooling (p=0.0317), subtype DSI (14.2%, p=0.0043), and chronic (p=0.7022). Hypnotic drugs were used by 6.9%(sd=1.3%) in the last month. The aim of this study was to evaluate the frequency and intensity of cerebral atrophy using CT scanning and the possible relation to corticosteroid therapy or disease in systemic lupus erythematosus (SLE) and to analyse the relationships between cerebral atrophy and activity disease and neuropsychiatric manifestations in lupus patients. We studied 107 consecutive SLE patients (American Rheumatology Association 1982 criteria) who were taking steroid drugs at the time and not selected for any particular manifestation (group 1). A complete clinical, neurological and laboratory evaluation was performed. The American College of Rheumatology's classification for neuropsychiatric manifestations and SLE disease activity index for activity were employed. Group 2 comprised 39 non-SLE patients with oral chronic steroid use (1 mg/k/day for more than 3 consecutive months); 50 normal individuals were the controls (group 3). There were no demographic differences between the groups. Brain CT was performed in all individuals and the frequency and the intensity (minimal, moderate and severe) of atrophy analysed, through well-defined measures and indices, by two neuroradiologists. Cerebral atrophy was significantly more frequent in groups 1 and 2 than in group 3, but with no significant difference between groups 1 and 2. The severity of cerebral atrophy was significantly higher in SLE patients (p<0.05), independent of steroid dose or duration of disease. In both groups no patient presented severe atrophy. Lupus patients with and without cerebral atrophy presented neuropsychiatric manifestations and activity disease in a similar proportion. The more frequent neuropsychiatric manifestation in lupus patients with cerebral atrophy was seizures (p<0.05). Chronic glucocorticoid therapy was responsible for cerebral atrophy, with a comparable incidence in both lupus and non-lupus patients compared to age and gender-matched normal subjects untreated with glucocorticoids. The disease activity was not related to cerebral atrophy in group 1 and seizures were the neurologic manifestation related to cerebral atrophy. The severity of the cerebral atrophy was independent of steroid dose, or duration of treatment. Moreover, the disease itself contributes to the severity of this process, but not to the development of cerebral atrophy.
