Se presenta un caso de embarazo heterotópico espontáneo, intrauterino y ovárico, atendido en la Clínica Santa Isabel de Lima. Paciente de 35 años de edad que acudió por retraso menstrual, sangrado vaginal escaso y prueba de embarazo positiva. La ecografía endovaginal mostró pequeño saco gestacional, con ausencia de ecos embrionarios. Se realizó legrado uterino el 01/11/2006, obteniéndose vellosidades con degeneración hidrópica no molar, en el examen histológico. Diez días después se realizó control ecográfico, por sangrado vaginal persistente y dolor abdominal, encontrándose una tumoración mixta en el ovario izquierdo, con una pequeña imagen ecolúcida en su interior. Se realizó laparotomía exploratoria, siendo el hallazgo hemoperitoneo de 200 mL y tumoración retrouterina alargada de color violáceo, friable, dependiente de ovario izquierdo, compatible con embarazo ectópico ovárico.
Objetivos: Determinar los beneficios de la inserción del dispositivo intrauterino (DIU) con levonorgestrel en leiomiomatosis uterina asociada a hipermenorrea. Diseño: Estudio descriptivo, retrospectivo. Participantes: Mujeres con leiomiomatosis uterina e hipermenorrea. Intervenciones: A 15 pacientes con leiomiomatosis uterina e hipermenorrea se les insertó DIU-levonorgestrel como una alternativa al manejo quirúrgico usual. Se incluyó pacientes con leiomiomas entre 2 y 5 centímetros de diámetro promedio que no fueran intracavitarios y con un número máximo de 4, con un seguimiento a 12 meses. Principales medidas de resultados: Reducción del tamaño del leiomioma y del volumen menstrual. Resultados: Una paciente expulsó el dispositivo al tercer mes de su inserción. En 8 de las 14 restantes (57,1%) se redujo el tamaño promedio del leiomioma entre 25 y 50% con respecto al tamaño inicial; en 5 de las pacientes (35,7%) el tamaño permaneció igual y en una (7,1%) creció. El volumen menstrual disminuyó de manera estadísticamente significativa en 85,7% (12 de las 14 pacientes) e incluso dos de las de este grupo entraron en amenorrea. Solo una paciente (7,1%) reportó aumento del volumen menstrual. Conclusiones: El grupo de estudio es pequeño, pero puede servir para considerar al DIU-levonorgestrel como una alternativa al manejo quirúrgico de pacientes con leiomiomas asociados a hipermenorrea.
El embarazo ectópico es una patología frecuente de encontrar para el ginecoobstetra, siendo posible que dicho embarazo tenga una diversidad de lugares de desarrollo extrauterino, algunas de ellas ni sospechadas. Se presenta un caso de embarazo ectópico cornual (muñón) con ausencia de trompa, debido a salpingectomía un año antes por otro ectópico y evidencia de ovulación por el ovario contralateral (presencia de cuerpo lúteo), incluyendo la trompa de este lado intacta, que lo convierte en un embarazo abdominal. Se realizó la resección del trofoblasto por laparoscopia, corroborando la patología el diagnóstico de embarazo ectópico. La evolución postoperatoria fue satisfactoria.
