RESUMOA presença da resistência à insulina e da adiposidade visceral na obesidade mórbida, contribui para a prevalência de Síndrome Metabólica (SM) na quase totalidade dos pacientes obesos, estando relacionada ao desenvolvimento de diabetes mellitus tipo 2 (DM2). Ao abordar mecanismos patogenéticos complementares, a cirurgia metabólica parece ser promissora em relação à reversão das anormalidades metabólicas que levam ao DM2 evidente. Pela análise dos estudos, tiveram maior probabilidade de atingir a resolução total do diabetes tipo 2, pacientes com diabetes pré-operatório de menos de 5 anos e aqueles com a forma mais branda da doença. É demonstrado que a cirurgia de ByPass gástrico (RYGB) pode promover melhor perda de peso, controle glicêmico e diminuição do uso de medicamentos hiperglicêmicos em pacientes diabéticos com IMC <35 kg/ m 2 , em comparação a pacientes recebendo terapia médica padrão. Em estudos comparando o tratamento cirúrgico e não cirúrgico da obesidade mórbida, a cirurgia bariátrica foi associada a maior perda de peso, maiores taxas de remissão de DM2 e síndrome metabólica, melhores perfis lipídicos, melhora na qualidade de vida e reduções substanciais das necessidades de medicamentos. Dessa forma, a cirurgia metabólica, é uma abordagem terapêutica que promete não apenas ser uma ferramenta de gerenciamento eficaz, mas também uma cura potencial para a diabetes tipo 2.
