As infecções do complexo maxillomandibular são corriqueiras na odontologia, podendo ser um quadro facilmente revertido, a depender da habilidade do profissional de diagnosticar e tratar precocemente, bem como da imunocompetência do paciente. Objetivou-se por este estudo relatar a conduta clínica de um caso de infecção odontogênica, com disseminação para o espaço fasciais secundários e evolução para fístula extraoral. Paciente homem, 49 anos. Ao exame físico apresentou extenso acúmulo de coleções purulentas e pontos de fístula intra e extraoral, tendo como foco infeccioso oriundo do elemento 47. Através da história da doença atual, exame clínico e tomográfico foi definido o diagnóstico de celulite de face com abscesso dentoalveolar crônico disseminado para os espaços fasciais infratemporal, temporal profundo, bucal, submandibular e cervical. A conduta terapeutica proposta foi a drenagem cirúrgica e ordenha das coleções purulentas, antibióticoterapia e exodontia do elemento dentário. Conclui-se que faz-se importante o diagnóstico e tratamento precoce, a fim de evitar a progressão para complicações mais severas, como a mediastinite, trombose em seio cavernoso, angina de Ludwig e fasciíte necrosante.
