Complications associated with acetate hemodialysis include hypoxemiaand imparirment of metabolic acidosis, these reach its lower point at one hour the of starting procedure. They constitute a disadvantage in the management of inestables or critical patients. In order to describe the evolution of arterial gas an acid-base states during venovenous hemofiltration (VVHF), we evaluated eight procedures realized in equal number of patients with uremia or acute pulmonar edema. The replacement solution was Ringer´s lactate. Neither hypoxemia, not variations in arterial pH or serum bicarbonate did occur during VVHF. We feel that hemofiltration is better alternative in the management of inestable or critical patients who require dialytic support. (Rev Med Hered 1994; 5: 33-37).
Con el objetivo de validar un micrométodo de determinación del CO2 total sanguíneo por titulación se realizaron dos estudios. El primero compara, en 50 adultos clínicamente sanos (GRUPO 1A), los valores de CO2 total obtenidos por el método de titulación convencional de Scribner con los del micromicrométodo. Además, se comparan los mismos valores en un grupo de 50 pacientes de programas de hemodiálisis crónica intermitente (GRUPO B). El segundo estudio compara los valores de CO2 total obtenidos por micromicrométodo de titulación con la determinación de la concentración arterial de bicarbonato por análisis de gases arteriales en 50 pacientes en hemodiálisis crónica intermitente (GRUPO 2). No encontramos diferencia significativas entre las determinaciones del CO2 total por ambos métodos de titulación, tanto en el GRUPO 1A (Scribner: 27.83 ± 2.44 mEq/L, micrométodo : 27.83 ± 2.37 mEq/L; p=ns, ANOVA) como en el 1B (Scribner: 19.70 ± 2.06 mEq/L, micrométodo: 19.42 ± 1.96 mEq/L; p=ns, ANOVA). La correlación entre los valores obtenidos por ambos métodos de titulación fue altamente signigicativa: GRUPO 1A: R=0.95, P<0.001; GRUPO 1B:r=0.78, p<0.001. También resultó estadísticamente significativa la correlación entre los valores comparados en el GRUPO 2 (r=0.74, p<0.001). Se remarca la importancia de informar el resultado del micrométodo como el promedio e tres lecturas distintas en la misma muestra. Recomendamos la determinación del CO2 total por micrométodo como método auxiliar clínicamente útil en la evaluación del estado ácido-básico.
