Durante un año fueron estudiados 61 pacientes consecutivos VIH positivos, con el fin de establecer la prevalencia de las manifestaciones oftalmológicas. Veintinueve pacientesestaban en estadío de SIDA (IV de la Clasificación CDC) y 32 eran portadores asintomáticos. En la primera evaluación, se encontró compromiso oftalmológico en 15/29 pacientes con SIDA (51.72%); siendo los hallazgos: microangiopatía retinal no infecciosa 27.6%, infecciones oportunísticas 20.68%; Sarcoma de Kaposi, 3.44% y atrofia óptica en 3.44%. De las infecciones oportunísticas encontradas en la primera visita, la más importante fue la retinitis por citomegalovirus (CMV) en 10.34%. En portadores asintomáticos, uno presentó microangiopatía retinal no infecciosa con presencia de manchas algodonosas retinales (3.1%). En el seguimiento, de los pacientes con SIDA cuyo examen inicial era normal, 2 desarrollaron microangiopatía y 3 retinitis por CMV; mientras que de los pacientes que al inicio tenían microangiopatía retinal, 5 desarrollaron retinitis por CMV en la evolución. También se observó un nuevo caso de Sarcoma de Kaposi conjuntival y uno de parálisis del VI par craneal y papilitis. Al término de este estudio habíamos acumulado 9 casos de retinitis por CMV (31%) y compromiso oftalmológico en 19/29 pacientes con SIDA (65.51%). De los 32 portadores asintomáticos, uno desarrolló retinitis por CMV, debutando como SIDA con una manifestación ocular. Se concluye que el compromiso oftalmológico en el SIDA es importante, por lo que es necesario un adecuado seguimiento oftalmológico en esta población.
Introducción: la fibrosis quística (FQ) es una enfermedad crónica, progresiva, con compromiso multisistémico y de alta mortalidad. Un diagnóstico temprano y un manejo multidisciplinario llevan a una mayor sobrevida y mejor calidad de vida. Objetivo: caracterizar los pacientes que fallecieron con diagnóstico de FQ en el periodo comprendido entre 2011 y 2017 en el Hospital Universitario San Vicente Fundación (HUSVF) de Colombia. Metodología: estudio descriptivo de series de casos, con recolección de la información de forma retrospectiva. Resultados: Se encontraron 168 pacientes con FQ en seguimiento en la institución durante el periodo del estudio, de los cuales 17 fallecieron (10.1%). Once muertes correspondieron a menores de 15 años (64.7%), hubo una distribución de género equitativa (mujeres 52.9% y hombres 47.1%). La mediana de edad al momento del diagnóstico fue de 3 años y la de muerte 12 años. El compromiso clínico más frecuente fue sinopulmonar y gastrointestinal. La hipertensión pulmonar se presentó en el 29.4%. Todos los pacientes tenían un grave compromiso funcional, nutricional y el 82,4% estaban colonizados por Pseudomonas aeruginosa. Ninguno tuvo manejo multidisciplinario y la mayoría presentaban un bajo nivel socieconómico. En 6 pacientes se utilizó BiPAP como medida paliativa y todos murieron por falla ventilatoria. Conclusión: la mortalidad por FQ en nuestra población se sigue presentando a edades tempranas. El diagnóstico de la enfermedad aún se realiza de forma tardía, comparado con países desarrollados. Hubo alta proporción de colonización bacteriana de la vía aérea y los pacientes presentaron un estado clínico y funcional grave antes de morir.
Las adenopatías son crecimientos de cadenas ganglionares que se presentan en diferentes patologías presentes en procesos infecciosos e infiltraciones neoplásicas o en estado de inmunosupresión en pacientes con serología positiva para el Virus de Inmunodeficiencia Humana. Se presenta el caso de paciente masculino de 47 años, que presenta desde hace tres meses, deposiciones diarreicas, líquidas abundantes de mal olor, tos seca, significativo deterioro del estado general, caquexia, cuadros febriles nocturnos y diaforesis. Se añadió epigastralgia urente, los últimos quince días, crecimiento ganglionar en cadenas cervicales bilaterales y disfagia. Paciente acude a hospital básico, donde realizaron un diagnóstico serológico con inmuno ensayo enzimático (ELISA) para del Virus de Inmunodeficiencia Humana, motivo por el cual fue transferido al Hospital Ambato para confirmación y tratamiento. Paciente al examen físico: caquéctico, deshidratado, taquicárdico, secreción retronasal blanquecina, placas blanquecinas a nivel de pilares amigdalinos anteriores, adenopatías cervicales bilaterales dolorosas, abdomen doloroso a nivel de epigastrio. Se complementaron los estudios confirmándose el diagnóstico de síndrome de inmunodeficiencia adquirida, leishmaniasis ganglionar y tuberculosis ganglionar. El presente caso tiene relevancia en el campo de la docencia – médica debido a que el paciente presentóadenopatías en cadenas ganglionares cervicales el médico debe realizar el diagnóstico diferencial entre patologías frecuentes y atípicas, ademásde cuadros infecciosos: virales, bacterianos incluyendo tuberculosis, micóticos y parasitarios, así como coinfecciones de etiología poco común que agravan el estado de inmunodepresión. Es importante tener la sospecha clínica para establecer el de adenopatías en pacientes inmunodeprimidos, a fin de que el diagnóstico sea oportuno y el tratamiento instaurado el más adecuado.
