OBJECTIVES:
The aim of this study was to evaluate the quantitative serum level of infliximab (IFX) as well as the detection of anti-infliximab antibodies (ATIs) in patients with Crohn's disease (CD).
METHOD:
Forty patients with CD under treatment at a tertiary center in southeastern Brazil were evaluated. Their use of infliximab was continuous and regular. We analyzed and compared the differences in the IFX and ATI levels between the patients with active CD (CDA) and those with CD in remission (CDR).
RESULTS:
There was no difference in the IFX level between the CDA and CDR groups (

>0.05). Eighty percent of all patients had IFX levels above the therapeutic concentration (6-10 μg/mL). Two (9%) of the 22 patients with active disease and four (22.2%) of the 18 patients in remission had undetectable levels of IFX. Four (66.6%) of the six patients with undetectable levels of IFX had positive ATI levels; three of these patients were in remission, and one had active disease. In addition, the other two patients with undetectable levels of IFX presented ATI levels close to positivity (2.7 and 2.8 AU/ml). None of the patients with therapeutic or supratherapeutic IFX levels had positive ATI levels.
CONCLUSIONS:
The undetectable levels of IFX correlated with the detection of ATIs, which was independent of disease activity. Immunogenicity was not the main factor for the loss of response to IFX in our study, and the majority of patients in both groups (CDA and CDR) had supratherapeutic levels of IFX.
Objective: Descrever sobre a importancia dos registros no prontuário do paciente, tendo em vista a considerável relevância do dever legal e ético dos profissionais da saúde, além de destacar a importância deste documento. Revisão Bibliográfica: Em decorrência do valor legal e ético existente no prontuário do paciente, o emprego da correta forma de preenchimento deve ser um dos preceitos legais empregados na formação profissional. O prontuário do paciente é um documento individualizado e sigiloso, amparado pelos preceitos éticos e legais, e os registros ali contidos são extremamente relevantes ao paciente e aos profissionais envolvidos nessa assistência. Desse modo, o prontuário expressa a centralização das informações, da progressão do paciente e assim deve-se empregar a real importância das informações registradas, recebendo atenção dos profissionais, transgredindo o simples registro. Considerações finais: Os profissionais devem entender a seriedade no preenchimento do prontuário do paciente, inserindo na formação básica dos mesmos elementos subsidiadores ao correto preenchimento deste documento, e ao final torná-lo adequado e em conformidade aos preceitos éticos e legais de seu preenchimento, guarda e descarte.
Objetivo: Identificar a atuação do Núcleo de Apoio Técnico ao Judiciário (NATJUS) nos requerimentos de pareceres referente à solicitação de internação em Unidade de Terapia Intensiva. Métodos: Trata-se de um estudo quantitativo que utilizou dados secundários sobre o requerimento de parecer no sistema do Núcleo de Apoio Técnico do Poder Judiciário, referentes às solicitações de internação em unidade de terapia intensiva e cuja data de criação do parecer era referente ao ano de 2020. Resultados: Foram encontrados 104 pareceres com o descritor “unidade de terapia intensiva” no ano de 2020. Observou-se que as maiorias dos pacientes eram do sexo masculino, e a média de idade era de 56 anos. Além disso, a maioria dos pareceres (76,92%) foi favorável às solicitações de internações, e o Estado de Tocantins foi o que mais solicitou pareceres ao núcleo de apoio técnico. Conclusão: Embora a judicialização se mostre crescente, observou-se baixo número de consultas dos Estados brasileiros diante das necessidades de internação em Unidade de Terapia Intensiva, e cujo parecer pode ofertar elementos técnicos e literários.
