Las pruebas de laboratorio tienen un impacto crítico en la toma de decisiones médicas. Sin embargo, en el ciclo, que inicia con la solicitud del médico y termina con la interpretación final, pueden ocurrir errores en cualquier etapa. La evaluación de las causas que generan estos errores y la toma de medidas para detectarlos y prevenirlos, antes de que causen daño, es crítico en el proceso analítico. Esto se logra a través de la gestión de riesgos (GR). La norma EP23A, Control de calidad en el laboratorio basado en la gestión de riesgos, del Clinical Laboratory Standards Institute (CLSI), introduce los principios de la GR para los laboratorios clínicos. Esta directriz permite desarrollar un plan de GR, individualizado. Este artículo resume los principios de la GR en los laboratorios clínicos.
Objetivo: Medir los tiempos (días) antes y después de la implementación de una unidad de mamovigilancia, para el seguimiento de las pacientes con sospecha mamográfica de cáncer, hasta su diagnóstico anatomopatológico o intervención clínica. Materiales y métodos: Se diseñó un estudio cuasi experimental pre-post, para comparar la mediana de los tiempos (días) en los intervalos de referencia y diagnóstico, sin la implementación de una unidad de mamovigilancia en el 2014-2015 y con su implementación en el 2017, en el Hospital IIISuarez Angamos - EsSalud. Se realizó un análisis no paramétrico con la prueba U de Mann-Whitney. Resultados: Se incluyeron 99 pacientes. El intervalo de referencia sin la unidad de mamovigilancia, tuvo una mediana de 37 días, en comparación a 33 días con la unidad implementada (p=0,003). Sobre el intervalo de diagnóstico, su mediana fue de 189 días sin la unidad y de 56 días con la unidad de mamovigilancia implementada (p<0,001). Conclusión: La implementación de una unidad de mamovigilancia disminuye los tiempos en días en beneficio del diagnóstico oportuno del cáncer en pacientes con sospecha mamográfica en la Red Asistencial Rebagliati-EsSalud.
El presente artículo especial tiene como propósito detallar las normas y regulaciones, elaboradas por instituciones peruanas, involucradas con la calidad analítica de los laboratorios cínicos. Además, refuerza la importancia de las responsabilidades de los gestores y profesionales de la medicina de laboratorio, en su rol de entregar resultados confiables y con calidad analítica, que permitirán una toma de decisiones clínicas por parte del profesional de la salud y así realizar una intervención oportuna al paciente, en beneficio de su diagnóstico, monitoreo o tratamiento.
Objetivo: determinar la prevalencia de albuminuria en una cohorte compuesta por pacientes atendidos en una red de salud de la seguridad social en Lima y determinar la asociación entre hipertensión (HTA) y/o diabetes (DM) y albuminuria. Materiales y métodos: Se diseñó un estudio descriptivo transversal con análisis de datos secundarios de resultados de laboratorio de pacientes tamizados en una red de salud de Lima, durante dos años (2013-2014). Se calculó el cociente albumina-creatinina en los exámenes de orina cuantitativo y automatizado. Se utilizó una prueba de chi-cuadrado para obtener el valor p, por nivel de albuminuria. Se utilizó un modelo de regresión logística univariado para obtener los odds ratios crudos y determinar la asociación entre la presencia de las condiciones (HTA o DM) y albuminuria, y se ajustó un modelo de regresión logística múltiple. Se incluyeron los intervalos de confianza al 95%. Resultados: se incluyeron en el estudio 13 842 pacientes. La prevalencia de albuminuria fue del 23,31%, en diabéticos del 23,93%, en hipertensos del 18,03% y en pacientes con ambas condiciones del 35,22%. Se demostró una asociación significativa entre diabetes e hipertensión y el riesgo de albuminuria (categorías A2 y A3). Conclusión: se observó una prevalencia global del 23,31% en pacientes diabéticos e hipertensos en nuestra red de salud. Este tamizaje benefició a los pacientes de esta red en la seguridad social; por lo tanto, implementar intervenciones sanitarias para el diagnóstico oportuno del daño renal evitaría futuros pacientes con enfermedad renal crónica.
