El propósito del estudio fue evaluar histológicamente la reparación ósea alveolar post-exodoncia en animales de experimentación usando una membrana colágena tipo esponja y un material de sulfato de calcio. Se utilizaron 24 cobayos (Cavia porcellus) a los que se les realizó la extracción de las primeras molares mandibulares y fueron divididos en dos grupos: uno con membrana colágena tipo esponja y otro con material de sulfato de calcio. La reparación alveolar fue comprada con un grupo control y evaluada a los 7, 14 y 30 días por el método histomorfométricode conteo diferencial de puntos para los siguientes factores: coágulo sanguíneo, tejido de granulación, matriz provisional, hueso mineralizado. El coágulo sanguíneo se encontró en mayor porcentaje en el tercio apical para el sulfato de calcio a los 7 días (60,57%), la matriz provisionalpara la membrana colágena a los 7 días (85,48%) y el hueso mineralizado para el sulfato de calcio a los 30 días (50,73%). No hubo diferencias en la reparación alveolar al utilizar membrana colágena comparada con el lado control, aunque al comparar el sulfato de calcio con el control fue mayor en este último en matriz provisional a los 14 días y en hueso mineralizado a los 7 días.En conclusión, no hubo diferencia histológica en la reparación ósea alveolar post-exodoncia al comparar la membrana colágena tipo esponja y el sulfato de calcio con sus respectivos alvéolos controles.
La rehabilitación oral de pacientes edéntulos totales o parciales con implantes dentales ha probado ser una modalidad de tratamiento predecible a largo plazo. Un prerrequisito para el éxito de los implantes es la disponibilidad de suficiente volumen óseo, donde serán necesariosprocedimientos para el incremento de reborde alveolar cuando poco volumen óseo exista. Los injertos óseos autólogos intramembranosos son usados en la restauración del volumen óseo y son considerados el gold estándar para la reconstrucción de los rebordes alveolares. El objetivodel trabajo fue mostrar una técnica modificada para la obtención de un injerto óseo en bloque de rama mandibular con el uso de una trefina. Un paciente masculino de 30 años fue admitido para una rehabilitación oral implantosoportada, al examen clínico intraoral se observó una deficienciadel reborde alveolar de la zona 22 (clase III de Seibert), por lo que se planificó la colocación de un injerto óseo de rama mandibular obtenido con una trefina para la reconstrucción tridimensional del reborde. La rehabilitación con implantes en zona estética, constituye un desafío enimplantología y el injertos de rama mandibular es una fuente de hueso autógeno para la reconstrucción alveolar. La obtención del injerto de rama mandibular mediante una trefina es una alternativa viable y ventajosa frente a la técnica convencional debido a la fácil obtención del injerto, a la baja morbilidad del sitio donador, la reconstrucción tridimensional de los defectos alveolares, así como una reducción del tiempo operatorio y la recuperación post operatoria.
Se sabe que luego de una extracción dentaria el proceso alveolar esta sujeto a continuo remodelado óseo, así mismo, con el incremento de la edad los procesos de reabsorción predominan sobre la nueva formación ósea, así la perdida de dientes da lugar a un incremento en la reabsorción del proceso alveolar y a cambios en su morfología. Con el paso de los años, diferentes sistemas de implantes han sido introducidos con una sustancial variación en el sistema de fresado o perforación, así como también en la técnica quirúrgica aplicada. Obtener estabilidad inicial durante la colocación de los implantes es en efecto, uno de los criterios fundamentales para obtener oseointegración y lograr esta estabilidad depende de la densidad ósea, la técnica quirúrgica utilizada y las características macroscópicas y microscópicas del implante utilizado. Así cuando existen deficiencias en el volumen del tejido óseo fundamentalmente en ancho y este no cumple con los requisitos mínimos exigidos para la obtención de estabilidad, las técnicas de expansión crestal deben ser tomadas en cuenta para la ubicación de los implantes con predicibilidad.
