Sarcopenic obesity is characterized by the loss of muscle strength, mass and muscle functionality and increased adipose tissue (obesity) according to different criteria and cut-off points. The prevalence of sarcopenic obesity among older adults is growing worldwide, and many factors are involved in its development. Diet and food security have been described as the main contributors to the development of obesity and sarcopenia. Food insecurity consists of limited or uncertain access to adequate and nutritious foods. This narrative review aims to summarize the existing data on food insecurity as a risk factor for sarcopenic obesity in the elderly.
A long-term complication of obesity is the development of type 2 diabetes (T2D). Patients with T2D have been described as having epigenetic modifications. Epigenetics is the post-transcriptional modification of DNA or associated factors containing genetic information. These environmentally-influenced modifications, maintained during cell division, cause stable changes in gene expression. Epigenetic modifications of T2D are DNA methylation, acetylation, ubiquitylation, SUMOylation, and phosphorylation at the lysine residue at the amino terminus of histones, affecting DNA, histones, and non-coding RNA. DNA methylation has been shown in pancreatic islets, adipose tissue, skeletal muscle, and the liver. Furthermore, epigenetic changes have been observed in chronic complications of T2D, such as diabetic nephropathy, diabetic retinopathy, and diabetic neuropathy. Recently, a new drug has been developed which acts on bromodomains and extraterminal (BET) domain proteins, which operate like epigenetic readers and communicate with chromatin to make DNA accessible for transcription by inhibiting them. This drug (apabetalone) is being studied to prevent major adverse cardiovascular events in people with T2D, low HDL cholesterol, chronic kidney failure, and recent coronary events. This review aims to describe the relationship between obesity, long-term complications such as T2D, and epigenetic modifications and their possible treatments.
Background The gap between the nutrition education provided to medical students and the nutrition competencies and attitudes needed for physicians to provide adequate nutrition care is a global concern. There is no universally accepted benchmark on nutrition competencies for doctors. The objective of this study was to establish, by expert consensus, the objectives of undergraduate nutrition medial education, the nutrition core competencies, and strategies for curriculum development in medical nutrition education. Methods We administered a Delphi survey to systematically gather the opinion of a panel of Latin American experts in nutrition. The survey questionnaire was constructed considering scientific literature by using a 5‐point Likert scale. Consensus was defined as >70% agreement on the importance of an item (Likert scale 4 and 5). Results A four‐round Delphi survey was conducted for this research. In the second, third, and fourth rounds, we validated a total of 130 competencies by consensus, which were distributed into four different thematic areas: (1) basic nutrition concepts, (2) public nutrition and nutrition prevention throughout the life cycle, (3) nutrition status and disease, and (4) nutrition care process. Conclusion The curricula for general physician education in medical school must include health promotion, prevention, and treatment of diseases related to nutrition. This goal can be reached by integrating ≤130 competencies into four different fundamental areas.
Because of their condition, patients with morbid obesity develop several histopathological changes in the liver, such as non-alcoholic fatty liver disease (NAFLD), non-alcoholic steatohepatitis (NASH), cirrhosis, and end-stage liver disease (ESLD). Hence, a liver transplant (LT) becomes an opportune solution for them. Due to many challenges during the perioperative and postoperative periods, these patients are recommended to lose weight before the surgery. There are many proposals to achieve this goal, such as intragastric balloons and many different bariatric surgery (BS) procedures in combination with a preparation diet (very-low-calorie diet, ketogenic diet, etc.). All the interventions focus on losing weight and keeping the continuity and functionality of the digestive tract to avoid postoperative complications. Thus, this review analyzes recent publications regarding the metabolic and pathophysiological impacts of BS in LT patients suffering from NAFLD-related cirrhosis, the effect of weight loss on postoperative complications, and exposes the cost-effectiveness of performing BS before, after, and at liver transplantation. Finally, the authors recommend BS before the LT since there are many positive effects and better outcomes for patients who lose weight before the procedure. Nevertheless, further multicentric studies are needed to determine the generalizability of these recommendations due to their impact on public health.
Introducción: La cineantropometría en deportistas es imprescindible para la valoración cuantitativa de su Composición Corporal (CC) y nutricional, llevándonos a una percepción personalizada permitiendo establecer objetivos y estrategias que convergen en el entrenamiento y la dieta, con el fin de conseguir el éxito deportivo. Objetivo: Describir las caracteristicas antropométricas, la CC y el somatotipo específico por posición de juego en futbolistas profesionales de la primera división del Ecuador, con el propósito de establecer patrones antropométricos, que permitan en un futuro referenciar a la población de futbolistas y que pueda ser comparado con otros equipos profesionales Método: Se analizaron las variables antropometricas según protocol ISAK, de 30 futbolistas profesionales masculinos de un equipo de primera de la liga ecuatoriana de fútbol, evaluándose el somatotipo según Heath-Carter y su CC cálculándose la Masa Grasa (MG) de Yuhaz modificada por Carter y el cálculo de la Masa Libre de Grasa (MLG) y Masa Muscular (MM) por Lee; empleándose una estadística descriptiva (media±desviación estándar (Ds)) a través de SPSS 25.0 aplicando t- Student bilateral pareada (two-tailed paired t test) (P<0.005). Resultados: Los valores medios obtenidos son estatura (179±5,1 cm), peso (78±7,1 Kg), y la edad (25±5 años), siendo de mayor edad los centrocampistas y los de mayor altura los porteros y delanteros. Con respecto al IMC se obtuvo un valor de 24,9±2,2, siendo el valor medio de porcentaje de grasa corporal (%GC) de 13,1±3,3%, teniendo un valor superior los porteros y los defensas en menor proporción; con MLG 45,2 ±1,7%, con MG (9,8±2,9 Kg) y MM (66±,8Kg). El somatotipo es mesomorfo balanceado en todas las posiciones de juego (1,8 - 5,4 - 1,8). Discusión: Según su posición de juego, no existe diferencia entre los somatotipos con y con una CC acorde a lo publicado en este deporte deporte practicado.
Algunos trabajadores mantienen estilos de vida no saludables que los hace propensos a tener un alto riesgo de diabetes tipo 2 (DT2). El presente estudio tuvo como objetivo evaluar el riesgo de DT2 (RDT2) en personal administrativo y docente de una universidad ecuatoriana y factores asociados. Estudio de corte transversal que evaluó el RDT2 según FINDRISC. Se consideró RDT2 alto si la puntuación ≥12 puntos. Se analizaron aspectos sociodemográficos, hábitos nocivos y saludables; se midieron el índice de masa corporal (IMC), el perímetro abdominal (PA) y la tensión arterial (TA). Se evaluaron las razones de prevalencia bruta (RPB) y ajustada (RPA) para identificar factores asociados con RDT2. De 311 participantes, edad media de 41,02 años (DE 10,1), 58,2% mujeres y 41,8% hombres. La prevalencia de RDT2 elevada fue mayor en mujeres respecto a varones (38,1% frente a 32,3%) y aumentó en mayores de 40 años [RPA 1.55 (1.11-2.15)], personas casadas [RPA 1.49 (1.07-2.05)], aquellos con actividad física moderada o menor [RPA 1.55 (1.11-2.15)], y en aquellos con PA elevado [RPA 2,41 (1,33 - 4,36)]. La edad, la baja actividad física y el PA, fueron factores asociados con una mayor prevalencia de RDT2. Se deben promover estilos de vida saludables, para incrementar la actividad física y disminuir el PA en trabajadores, para reducir el riesgo de DT2. Introducción: la sarcopenia es una condición que se puede asociar con la edad, caracterizada por la declinación funcional y estructural del músculo esquelético estriado. La suplementación con proteína, en combinación con ejercicio de resistencia, es el tratamiento principal para contrarrestar la sarcopenia o dinapenia en adultos mayores. El objetivo de este estudio es evaluar el efecto de la suplementación con proteína aislada de soya y el ejercicio de resistencia sobre la fuerza muscular y composición corporal en adultos mayores, que viven en una comunidad urbana marginal de Ecuador. Métodos: se realizó un estudio cuasiexperimental, con una duración de 12 semanas en 70 adultos mayores (65 años o más) con dinapenia o baja masa muscular. Los participantes recibieron 10 g de proteína aislada de soya al día y realizaron entrenamiento de fuerza. Se realizó un análisis estadístico descriptivo. Las variables se probaron con T de Student, U de Mann Whitney y Chi cuadrado. Resultados: se encontró un aumento estadísticamente significativo en la fuerza prensil de mano en mujeres (17 [15,5-21] kg frente a 20 [16,7-23] kg; p = 0,0001), pero no en hombres; y aumento de la masa muscular (39,1 [35,6-44,3] kg frente a 39,9 [35,9-45,5]; p = 0,024) en ambos sexos. La masa grasa corporal total y la grasa visceral disminuyeron en los dos grupos. Conclusión: la suplementación con proteína aislada de soya junto con ejercicio de resistencia durante 12 semanas mejoró la fuerza muscular en mujeres y la composición corporal en hombres y mujeres en la muestra estudiada.
