Dental diastema is a space or absence of contact between two consecutive teeth, which may represent an aesthetic embarrassment for the patient when smiling. For the aesthetic resolution of diastema some factors should be evaluated as, age of the patient, dental structure conservation, reversibility of treatment, cost and longevity. For closing diastema, for a long time, the clinical alternatives were the use of orthodontics and fixed prostheses. Currently, with composite resins, it is possible to reproduce natural characteristics of the teeth with a direct, reversible restorative technique, without the wear of healthy dental structure, with affordable cost, able to provide satisfactory aesthetic results. The aim of this study is to report a case of a patient diagnosed with inter incisor diastema and to present a literature review. Patient, male, 70 years old, was attended at oral clinic of Ingá University Center - Uningá, complaining about the aesthetics of his smile. Clinically, was observed the presence of a diastema between upper incisor, fracture of incisal edge of central incisors and poor dental positioning. A reverse planning of the case was carried out through diagnostic waxing. With aid of a silicone guide, the diastema was closed through reanatomizations in composite resin. After the proposed treatment, was possible to obtain a satisfactory aesthetic in the patient's smile. The closure of diastema, with the use of composite resins was able to provide a better contour to the teeth, closing the spaces present, resulting in an aesthetic and pleasant smile.
O desenvolvimento de materiais restauradores estéticos e duráveis e de técnicas de cimentação adesiva ampliaram as possibilidades de tratamento para dentes posteriores com ampla perda de estrutura. Atualmente, o clínico pode optar por técnicas diretas e indiretas, estas últimas ainda, podem ser restaurações parciais ou totais, as conhecidas coroas unitárias. Para elementos com grande perda de estrutura dentinária sob as cúspides remanescentes, a indicação de restaurações de cobertura parcial ou total podem se sobrepor, um vez que não são claramente abordadas na literatura. O preparo para coroas totais é mais uniforme e não exige a elaboração de um projeto específica para cada paciente. Assim observa-se uma preferência pelos clínicos em se utilizar a coroa total, muitas vezes ás custas de estrutura dental sadia, que poderia ser preservada. O objetivo desta revisão da literatura é discutir critérios de indicação para restaurações indiretas do tipo onlay/overlay e coroas totais, além de esclarecer as características ideais dos preparos para estas modalidades restauradoras. Considerando que ambos os tratamentos apresentam sucesso clínico em longo prazo, indicação de um ou outro tratamento deve ser baseada nas características da estrutura dentária remanescente e na vitalidade pulpar do elemento, optando-se sempre pela intervenção mais conservadora.
Este estudo avaliou a resistência de onlays de resina nanocerâmica, silicato de lítio com zircônia e dissilicato de lítio. As onlays foram submetidas à ciclagem mecânica por 1.500.000 ciclos, sob cargas de 0 a 250 Newtons a 2 Hertz. A cada 125.000 ciclos os espécimes foram avaliados e os dados de sobrevivência foram submetidos ao teste de Qui-quadrado, o número de ciclos até a falha analisado pelo teste de Kruskal Wallis e teste de Dunn. Concluímos que as onlays apresentaram excelentes taxas de sobrevivência, sem diferença estatística. Restaurações em dissilicato resistiram mais que as de silicato de lítio com zircônia.
A tecnologia CAD/CAM é utilizada na odontologia principalmente na produção de estruturas protéticas. Restaurações indiretas são planejadas e fabricadas com o auxílio do computador diminuindo a influência do processo manual executado pelo técnico em prótese dentária (TPD). Além do uso de materiais padronizados, de qualidade industrial, os sistemas CAD/CAM podem ser uma ferramenta útil no diagnóstico e planejamento do tratamento, além de permitir a fabricação e restaurações de ótima qualidade em menor tempo. As restaurações CAD/CAM apresentam desempenho clínico compatível com as restaurações convencionais e esta tecnologia pode ser incorporada na pratica clínico com poucas mudanças. Este estudo mostrará o funcionamento e as aplicações da tecnologia CAD/CAM em odontologia através de uma revisão de literatura, além dos materiais que podem ser utilizados. Esta tecnologia apresenta uma gama enorme de possibilidades e traz inúmeros benefícios tanto para o cirurgião dentista quanto para o TPD. Porém, é preciso conhecer as ferramentas de cada sistema para se beneficiar de suas facilidades, pois, aliar a manufatura computadorizada a técnicas manuais podem garantir melhor acabamento e estética.
