ResumoO objetivo desta pesquisa foi avaliar a presença da relação entre os sonhos e craving em alcoolistas nos três primeiros dias de desintoxicação em unidades de internação hospitalar.Verificou-se se aqueles que tinham o craving aumentado relatavam sonhos com o tema "álcool" e analisou-se a qualidade do sono desses sujeitos. Foi um estudo transversal, de associação entre variáveis. A amostra foi de 77 sujeitos adultos do sexo masculino, dependentes de álcool, sem comorbidades clínicas ou psiquiátricas e não-dependentes de outras substâncias psicoativas, salvo a nicotina. Os instrumentos foram: entrevista estruturada; escala de avaliação do craving; questionário de avaliação do sono e dos sonhos;Mini-Mental State Examination e questionário Short-Form Alcohol Dependence Data. Quanto à fase inicial do sono, 67,6% considerou no mínimo satisfatória, 80,5% emitiu a mesma opinião quanto ao seu final, porém apenas 22,1% nunca apresentou interrupções durante o sono. Sonhar com álcool não foi um comportamento freqüente (27,3%), e a média de pontuação do craving foi "fraca", havendo associação entre sonhar com álcool e um aumento no craving (p < 0,001). Os sonhos podem ser mais bem aproveitados pelos profissionais da dependência química, devendo ser utilizados elementos sinalizadores de uma "situação de risco" nas técnicas de prevenção à recaída. ResumoO craving pode ser entendido como um desejo de utilizar uma substância. Este estudo pretende verificar características do craving em alcoolistas, nos primeiros três dias de abstinência, comparar sua intensidade com a do período de consumo do álcool e verificar se está associado com a gravidade do alcoolismo. A amostra foi de 77 homens alcoolistas, internados para desintoxicação. Foi utilizada a Escala de Avaliação do Craving e o Short-Form Alcohol Dependence Data. Destes sujeitos, 58,2% nunca sentiu vontade de utilizar bebidas alcoólicas nos três primeiros dias de abstinência. Quanto ao período antes da internação, 90,9% relatou apresentar craving, mais intenso durante essa fase de consumo do que na abstinência. Obteve-se correlação positiva entre o craving e a gravidade da dependência. Conhecer as características do craving de alcoolistas torna possível ampliar as técnicas de prevenção de recaída. This study intends to check craving characteristics in alcoholics during the first three days of abstinence syndrome, compare its intensity to the one during the alcohol consumption period, and verify if craving is associated with the degree of alcohol dependence. The sample is formed by 77 male alcoholics interned for detoxication. Scale for Craving Evaluation and a Short-Form Alcohol Dependence Data were applied. 58.2% of the participants never will to drink alcoholic beverages on the first three days of abstinence. Concerning the time before hospitalization, 90.9% stated feeling craving and during consumption stage it was more intense than during abstinence. A desintoxicação do álcool, é importante destacar, é uma das mais difíceis, em virtude dos sintomas físicos da sua característica síndrome de abstinência, que inclui sudorese, hipertensão arterial, taquicardia, tremores, convulsões, podendo, até mesmo, ocorrer um quadro de Delirium Tremens, quando são acrescidos delírios e alucinações. Clín. 33 (4); 195-203, 2006 ResumoEsta pesquisa tem como objetivo comparar a motivação para mudança em dependentes de álcool internados em hospitais gerais para tratamento de doença clínica com pacientes alcoolistas que buscaram tratamento em unidades especializadas. Trata-se de um estudo transversal, de comparação entre dois grupos. A amostra teve 91 sujeitos: 42 com doença clínica associada à dependência do álcool (grupo 1) e 49 dependentes do álcool internados em unidades especializadas para o tratamento da dependência química (grupo 2). Os instrumentos utilizados no estudo foram: entrevista estruturada, o The Alcohol Use Disorder Test (AUDIT), o Short-Form Alcohol Dependence Data (SADD), o The Drinker Inventory of Consequences (DrInC) e a University of Rhode Island Change Assessment Scale (URICA). Os resultados demonstraram haver diferença significativa entre os grupos 1 e 2 no estágio da ação (p = 0,006), não sendo encontrada diferença nos estágios de pré-contemplação (p = 0,098) e contemplação (p = 0,342). Foi encontrada diferença significativa entre os grupos apenas no estágio de manutenção (p = 0,000). Os pacientes internados em hospitais gerais apresentaram-se menos motivados para mudança do comportamento dependente do que os internados em unidades especializadas em dependência química. Conclui-se, portanto, a importância de trabalhar a motivação para mudança do comportamento dependente em pacientes alcoolistas internados em hospitais gerais por complicações físicas.
Este trabalho tem por objetivo realizar uma revisão teórica a respeito do sono e dos sonhos de alcoolistas de acordo com a Terapia Cognitivo-Comportamental, aprofundando aspectos cognitivos e neurológicos. Na medida em que esta é a abordagem psicoterápica mais indicada para dependência química, os sonhos, utilizados na Terapia Psicanalítica, tiveram de ser estudados sob o enfoque cognitivo-comportamental, para que pudessem ser aproveitados na clínica com estes pacientes. As alterações no sono de alcoolistas, por exemplo, pode servir como alerta quanto a um risco de recaída, o que indica que a avaliação do sono não pode ser desconsiderada pelos profissionais, durante o tratamento destes pacientes. O relato dos sonhos, por sua vez, pode ser um instrumento terapêutico muito valioso por oferecer pistas a respeito do quanto o craving está presente naqueles que estão tentando manter-se em abstinência. Se os profissionais forem bem sucedidos quanto à identificação precoce de uma situação de risco como o craving, poderão ser mais efetivos quanto à prevenção da recaída. Conclui-se quanto à necessidade de que sejam feitas novas pesquisas a respeito dos sonhos de acordo com esta abordagem teórica, bem como avaliada a sua aplicação na área da dependência química.
