RESUMO: Objetivo: descrever as orientações de alta pelo enfermeiro ao paciente pós-transplante renal. Método: trata-se de um estudo qualitativo descritivo. Foram realizadas entrevistas semiestruturadas e observações sistemáticas não participantes com três enfermeiros de um serviço de transplante renal de Curitiba-PR, de julho a setembro de 2010. Os discursos foram submetidos à análise de conteúdo, emergindo três categorias. Resultados: a primeira categoria descreve quais são as orientações fornecidas pelo enfermeiro ao paciente no momento da alta. Na segunda categoria são abordados aspectos da comunicação como competência fundamental a este profissional para o momento das orientações de alta e na terceira discorre-se acerca do conhecimento científico como alicerce para as orientações de alta pós-transplante renal. Conclusão: a prática educativa desenvolvida pelo enfermeiro no momento da alta pós-transplante é alicerçada, sobretudo, na comunicação e no conhecimento científico. Method: this is a qualitative descriptive study. Semi-structured interviews and non-participating systematic observations were performed with three nurses from the renal transplantation service of Curitiba-PR, from July to September of 2010. The speeches were submitted to content analysis, through which three categories emerged. Results: the first category describes the orientations given by the nurse to the patient at discharge. The second category approaches communication aspects as a core competence of this professional for the moment of discharge guidance. The third category deals with the scientific knowledge as a foundation for the discharge guidance of postkidney transplant. Conclusions: the educational practice developed by nurses at the time of post-transplant discharge is based mainly on communication and scientific knowledge.
